Agenda Item Text:
Approve revised Amendment 4 to Intergovernmental Agreement (IGA) ADHS18-177676 between Cochise County and the Arizona Department of Health Services (ADHS) for Immunization Services, to revise the Price Sheet from $125,000 to $25,000.
Background:
This amendment was originally presented to the Board of Supervisors and approved at the February 23, 2021 regular meeting. The original amount of the amendment was $125,000, however, this amount was in error. ADHS has corrected the amount to $25,000. We had not yet received a purchase order from the State and no funds had been expensed for this award.
