Approve Amendment 2 to Contract CTR050594, Health Start Program, between the Arizona Department of Health Services (ADHS) and Cochise Health & Social Services, in the annual amount of $353,000, effective July 1, 2022, through July 5, 2023.

Background:

Cochise County Health and Social Services has received the Health Start grant for over 35 years which allows us to promote healthy births and healthy families among Cochise County residents.  The Health Start Program serves pregnant women and families with children under age two. The purpose is to reduce pregnancy and post-partum risks through a community health worker model where trained community members act as a guide and mentor using an evidence-based curriculum called “Partners for a Healthy Baby” to support and educate parents about how to raise healthy children.  Pregnant women and parents receive guidance and information including what to expect during pregnancy, breastfeeding education and support, nutrition for children and pregnant women, screening for child development, activities to support healthy development and home health and safety.  These services are provided via home visits. However, at this time we are conducting visits via a hybrid model, seeing our participants either in their home or at a CCHSS office.
 
The Health Start Program, utilizing 5.5 staff positions, serves approximately 250 families, providing an average of 1,500-2,000 intervention visits, in large and small communities in every area of Cochise County. 

This contract was formerly titled ADHS IGA 15-096693.

Impact of NOT Approving/Alternatives:

Not approving this grant will result in the termination of this value-added program to at-risk pregnant women and children under age of two in Cochise County
