Approve CTR070113 Amendment 4, CDC Overdose Data to Action (OD2A), between the Arizona Department of Health Services (ADHS) and Cochise Health & Social Services (CCHSS) amendment changes.
Background:
The Overdose Prevention IGA, CTR070113 A3, beginning FY 2025 was approved on 10/7/2025.
Amendment 4 includes administrative and technical revisions to the contract, including updates to the contract term, budget transfer limitations, required federal award documentation, and related exhibits. These changes are compliance-based and do not alter program scope, services, or funding for the current fiscal year.
This is a grant-funded cost reimbursement program through the ADHS in the amount of $83,636. Funds are 100% federally funded. This IGA currently funds .80 FTE.
Amendment 4 does not change the current award amount.

Department's Next Steps (if approved):
Your approvals are respectfully requested. Program will continue to distribute Naloxone, provide trainings, coordinate transportation services, and promote local service providers.
