(a) Request to increase Apportionment Supplement for employee Cynthia Fey to $2.8846 per hour ($6,000 annually) from fund 180-4892-1060;
(b) Request to add Forfeiture Supplement for employee Cynthia Fey of $1.4423 per hour ($3,000 annually) from fund 180-4852-1056;
(c) Request to add an Apportionment Supplement for employee Dev'n Morrison of $0.7212 ($1,500 annually) from fund 180-4892-1060;
(d) Request to add an Apportionment Supplement for employee Amanda Cox of $0.7212 ($1,500 annually) from fund 180-4892-1060; and
(e) Request to add an Apportionment Supplement for employee William Flannery of $0.6731 ($1,400 annually) from fund 180-4892-1060.
