BACKGROUND

On December 17, 2015, the Building Department received a complaint from East Coast Lumber & Supply Co.  that American Lighting Maintenance, Inc.  failed to obtain a final inspection for and close out permit 11-2946.  They advised that they have made several calls and have not received a response back.
 
On December 28, 2015, American Lighting Maintenance, Inc., attempted to renew the permit on-line, however their license had expired and the renewal application was voided out.  The contact person at American Lighting and Maintenance advised permitting staff that it would take a day or two to get it done.
 
On December 30, 2015, American Lighting Maintenance, Inc., received our notice that a complaint had been filed against them.
 
As of February 1, 2016, a formal response has not been submitted by American Lighting Maintenance, Inc., and the permit has not been renewed.

FINDINGS

1 Investigator Coss has determined that American Lighting Maintenance, Inc., allowed permit 11-2946 to expire without obtaining approval for the final inspection.
2 Investigator Coss alleges that the following sections have been violated; 5-52(13)

ALLEGED CHARGES:

5-52 (13) Proceeding on any job without obtaining the applicable building department permits and inspections.

RECOMMENDATION:

To be determined.
