The Urban County Program requests approval of Subrecipient Agreement with Nuestra Clinica Del Valle (Health Services) in the amount of $20,000.00 for Urban County Program Year 23 (2010).
BACKGROUND
All funding will be utilized for the payment of primary health care and dental services, which include support services of pharmacy, radiology, laboratory, and sub-specialty referrals for children and adults. The Agreement shall be effective on July 1, 2010. (See the attached exhibits).
