a. Acceptance and approval of Change Order No.3 decrease in the amount of ($28,025.90) to reflect final quantity adjustments and an increase of 3 calendar day;

b. "Certificate of Substantial Completion" reflecting the Punch List date of March 27, 2012;

c.  Application for Final Payment in the amount of $71,647.57;

d.  Application for (Retainage) in the amount of $32,493.33;

e. "Certificate of Construction Completion" reflection the completion date of March 30, 2012.

With authorithy for County Judge or Court member to execute docment(s).
