Requesting Ratification for Form A: 2017 Renewal Application Special Supplemental Nutrition Program for the Women, Infant and Children (WIC).
BACKGROUND
The WIC Form A: FY17 Renewal Application Form was due on June 1, 2016. The Form A is part of the organizations contractual agreement with DSHS.
However, the Contract Manager had the wrong email address for Clarissa Ramirez, new WIC Director. She never received the email.
Ms. Deborah Fischer from the Auditors received the email from State on June 7, 2016 and forwarded it to Ms. Ramirez. Ms. Ramirez, submitted the application on June 7, 2016
