CAPTION

“Subject to Compliance with HB 1295”, the Urban County Program is requesting approval to purchase Equipment from Henry Schein, Inc. for the Hidalgo County Health Facility Project in the amount of $8,027.22.  UCP Fiscal Year 33 (2020) CDBG-CV Other Public Services funds will be utilized.

BACKGROUND

Three written quotes were requested, three were received and reviewed.  The lowest responsible quote was received from Henry Schein, Inc.  This is a Publicly Trade Company and has provided an unsworn declaration instead of a HB 1295 form. The equipment will be utilized at the Hidalgo County Health Facility.  Delivery is expected within 60 days of receipt of purchase order.  (Price Quotation form & Quotes are attached).
