1. Application for Payment No.2 (Final) in the amount of $37,646.10;
2. Application for Release of Retainage Payment in the amount $5,804.40;
3. Requestiong approval of Change Order No.3 (as reviewed by Raul Sesin, PE) with Total Commitment, LLC, contracted vendor for "North Tower Road Crossing Drainage Improvement Project" reflecting an additional 5 calendar days in a new total of 160 calendar days due to inclement weather days (of recent rains, ground conditions-too wet to conduct work within the project area) with authority for County Judge to execute document.
4. Requesting approval of "Certificate of Substantial Completion" reflecting the date of 10/25/2010.
