Request to remove fixed asset from department inventory list with detailed description of asset attached as "Exhibit A" Inventory Listing

BACKGROUND

Asset 56075, Apple ipad 4G 16GB Black BZN with ipad cover was purchased by the Office of Emergency Management with 2010 SHSP funding. The item was purchased to program and update communication radios out in the field as part of a communication project called "RAPID". It would provide the capability of tracking assets on scene as well as weather conditions during a response. The item was purchased and listed under the OEM department inventory and loaned to the Fire Marshals Office under a Property Permanent Loan Form on 8/28/12 (Exhibit B). The "RAPID" system was returned to OEM on 7/29/16. After review of the Permanent Loan Form, the ipad and cover were not located.
