Subject to Compliance with HB 1295” the Urban County Program is requesting approval of Supplemental Agreement No.1 to the Professional Engineering Services Agreement with SAMES, Inc. The original contract amount was $41,535.00 and will be increased by $29,405.00, for a total contract amount of $70,940.00 utilizing HOME FY’20 and FY’21 Rehabilitation Project funds, and also requesting approval of Work Authorization No. 2 for additional surveying, setback variance and housing inspection services.

SAMES Inc. submitted a setback variance and housing inspection proposals to amend their Professional Surveying Service agreement for FY’20-FY’21 Rehabilitation Project.  The amendment will allow for additional setback variance and housing inspection services added to the project.  Attached for review is the Supplemental Professional Services Agreement, Exhibits and Work Authorization No.2 as reviewed by District Attorney’s office, Certificate of Insurance and HB 1295 Forms.
