Amendment No.2 to IGA (Contract No. CTR060581) with Arizona Department of Health Services

Background Information

On October 4, 2022, the Board of Supervisors approved Intergovernmental Agreement Contract No. CTR060581 with the Arizona Department of Health Services (ADHS) to provide immunizations from July 1, 2022, to June 30, 2027, in the amount of $95,045.

Evaluation

The Board of Supervisors approved the IGA contract CTR060581 on October 4, 2022.

Amendment No. 1 to Intergovernmental Agreement Contract No. CTR060581 to amend the price sheet to reflect additional activities for expending the remaining contract budget until June 30, 2026, approved by BOS on April 21. 2026.

Amendment No. 2 to Intergovernmental Agreement Contract No. CTR060581 extends the contract through June 30, 2027.

Conclusion

Approval of Amendment No. 2 to Intergovernmental Agreement Contract No. CTR060581 to extend the Price Sheet Budget Period until June 30, 2027.

Recommendation

The Public Health and Community Services Department Director recommends that the Board of Supervisors approve Amendment No. 1 to Intergovernmental Agreement Contract No. CTR060581 to amend the Price Sheet Budget Period to reflect additional activities for expending the remaining contract budget until June 30, 2027.
