CALL TO ORDER
Vice-Chairman Stewart called the Special Meeting of the Police Public Safety Personnel Retirement Board meeting to order at 2:00 p.m.

Discussion and possible action on the application and medical records for Accidental Disability - Jason Mattie (Last heard on June 24, 2021)
The Board acknowledged receipt of the updated IME report.
Ms. Sorensen reviewed the questions on the Accidental Disability Questionnaire with the Board.
The Board noted that the additional IME requested at the June 24, 2021 meeting could be cancelled.

MOTION BY Boardmember Jay Mathias, SECONDED BY Boardmember James Seymore to ACCEPT the new IME report including the PSPRB form and APPROVE the Application for Accidental Disability Retirement for Jason Mattie. The motion carried as follows:
