CALL TO ORDER
Chair Kaino called the Police Public Safety Personnel Retirement Board meeting to order at 8:00 a.m.

APPROVAL OF MINUTES
Approve the draft minutes of the October 16, 2025 Police Public Safety Personnel Retirement Board meeting.
 

MOTION BY Boardmember Teresa Kuleff, SECONDED BY Vice Chair Jay Mathias to APPROVE the draft minutes of the October 16, 2025 Police Public Safety Personnel Retirement Board meeting. The motion carried as follows:

AYE:
Chair Laura Kaino, Vice Chair Jay Mathias, Boardmember Jason Costello, Boardmember Jason Bayer, Boardmember Teresa Kuleff

Passed - Unanimously

The Board moved into Executive Session for Items 2 and 3 at 8:02 a.m.

Review, discussion and possible action on the applications and medical records for entry into the Public Safety Personnel Retirement System.

MOTION BY Boardmember Jason Bayer, SECONDED BY Vice Chair Jay Mathias to ACCEPT the applications for Terry Graber, Dustin Lay, with pre-existing conditions, and Hunter Hershberger, with pre-existing conditions pending clarification of medical records for entry into the Public Safety Personnel Retirement System. The motion carried as follows:

Review, consideration and possible action on the application and medical records for Accidental Disability — Jared Jordan
The applicant was not present.
Without objection, the Board reconvened the Police Public Safety Retirement Board meeting at 8:25 a.m.
 

MOTION BY Boardmember Jason Bayer, SECONDED BY Boardmember Jason Costello to ACCEPT the accidental disability application and to send Jared Jordan to Independent Medical Examination (IME). The motion carried as follows:

Review, consideration, and possible action on the application and medical records for Accidental Disability — Scott Preston (continued from October 16, 2025)
The applicant was present, and verbally waived confidentiality. 
 

MOTION BY Boardmember Jason Costello, SECONDED BY Vice Chair Jay Mathias to APPROVE the application for accidental disability for Scott Preston. The motion carried as follows:

Lesli Sorensen, Board Attorney, reviewed the Accidental Disability Questionnaire with the Board. 

1. Did the employee file the application after the disabling incident, or within one year of ceasing to be an employee?  Yes.
2. Did (or will) the employee terminate by a reason of disability?  Yes.
3. Did employment terminate based on a disciplinary issue?  No.
4. If the member's period of DROP has ended, if applicable, did (or will) the employee terminate by a reason of disability?  Not applicable. 
5. Is the employee still working a position within their job classification that the local board considers a reasonable range of duties position?  No.
6. Has the employee refused a position within their job classification that the local board considered a reasonable range of duties?  No.
7. Did the injury or condition occur prior to the current PSPRS membership date?  No.
8. Was the injury or condition the result of an event incurred during the performance of the employee's duty?  Yes.

The Board acknowledged and thanked Mr. Preston for his years of service. 
 
EXECUTIVE SESSION

Per ARS §38-431.03, upon a public majority vote of the members constituting a quorum, the Goodyear Police Public Safety Personnel Retirement Board may hold an executive session for any item on the agenda.

MOTION BY Boardmember Teresa Kuleff, SECONDED BY Boardmember Jason Bayer to CONVENE an Executive Session. The motion carries as follows:

INFORMATION ITEMS

Leslie Sorensen, Board Attorney, reported that recent legislative changes to return-to-work provisions will require amendments to the Board’s Rules of Procedure. She noted a joint meeting to review the rules of procedures and annual actuarial reports will be held in March 2026.

Jasmine Pernicano, Board Secretary, reminded the Board of the upcoming PSPRS training.
 
FUTURE MEETINGS

The next Police Public Safety Personnel Retirement Board meeting is tentatively scheduled for December 18, 2025.
 
ADJOURNMENT
There being no further business to discuss, Chair Kaino adjourned the meeting at 8:38 a.m.
