CALL TO ORDER
Chair Kaino called the Police Public Safety Personnel Retirement Board meeting to order at 8:00 a.m.

APPROVAL OF MINUTES

Approve the draft minutes of the April 16, 2026 Police Public Safety Personnel Retirement Board meeting.
 

MOTION BY Boardmember Jason Costello, SECONDED BY Boardmember Teresa Kuleff to APPROVE the draft minutes of the April 16, 2026 Police Public Safety Personnel Retirement Board meeting. The motion carried as follows:

Review, consideration, and possible action on the application and medical records for Accidental
Disability — Michael Guilbault

Michael Guilbault, the applicant was present for the meeting. 

Lesli Sorensen, Board Attorney, verified that Mr.Guilbault did not waive his right to confidentiality, and therefore, the Board held any further discussion in Executive Session. 

The Board entered into Executive Session for Item 2 at 8:02 a.m.

Without objection, the Board reconvened the Police Public Safety Personnel Retirement Board meeting at 8:27 a.m.
 

MOTION BY Boardmember Jason Bayer, SECONDED BY Boardmember Jason Costello to ACCEPT the application for accidental disability for Michael Guilbault and appoint a medical board to conduct an Independent Medical Examination (IME) following receipt of additional medical records. The motion carried as follows:

Presentation, discussion, and appropriate action on the Intergovernmental Agreement between the Department of Public Safety (DPS) and Goodyear Police Local Boards.
Lesli Sorensen, Board Attorney, presented information on the Intergovernmental Agreement (IGA).

Chair Kaino clarified that City of Goodyear City Attorney Roric Massey also reviewed the IGA.

No additional discussion was held. 
 

MOTION BY Boardmember Jason Bayer, SECONDED BY Vice Chair Jay Mathias to APPROVE the Intergovernmental Agreement between the Department of Public Safety (DPS) and City of Goodyear Public Safety Personnel Retirement System Local Boards involving an accidental disability application for Rebecca York. The motion carried as follows:

Passed - Unanimously

Review, consideration, and possible action concerning the Accidental Disability application, medical records, and related documentation for Rebecca York, PSPRS Employer Arizona Department of Public Safety, as provided for in the Intergovernmental Agreement (IGA).
Rebecca York, the applicant was present for the meeting. 

Lesli Sorensen, Board Attorney, verified that Ms. York did not waive her right to confidentiality, and therefore, the Board held any further discussion in Executive Session. 

The Board entered into Executive Session for Item 4 at 8:33 a.m.

Without objection, the Board reconvened the Police Public Safety Personnel Retirement Board meeting at 10:25 a.m.
 

MOTION BY Boardmember Jason Bayer, SECONDED BY Boardmember Jason Costello to TABLE the application, pending receipt of additional information and medical records. The motion carried as follows:

Per ARS §38-431.03, upon a public majority vote of the members constituting a quorum, the Goodyear Police Public Safety Personnel Retirement Board may hold an executive session for any item on the agenda.
