ITEM:

Discussion and possible action on any and all matters regarding the Employee Benefit Plan for the fiscal year 2018-2019 as prepared and presented.  (Olivia Jenkins, Human Resources)

SUMMARY:

At the April 2018 meeting of the Trust, the Board of Trustees approved the costs of the plan for this fiscal year and approved EBSO as the Third Party Administrator since Mayo got out of the business of third party administration.

There were the approved costs of the plan approved in April,  shown here and also in the first attachment.

EBSO reviewed the plan and made minor substantive additions required, shown here and in the second attachment.

 Other changes to include grammatical changes.    

(Staff checked into what Amino Acid-Based formula is and this is the explanation found on Wikipedia:  "Amino acid-based formula, also known as an elemental formula, is a type of hypoallergenic infant formula made from individual amino acids. Amino acids are the building blocks of protein and together they form the protein requirements in formula needed for growth and development. The amino acids are in the simplest form, making it easy for the body to process and digest. Neocate, EleCare and Nutramigen Puramino are the more commonly encountered brands of amino acid-based formula in the US.")

EBSO will be handling and processing all medical and dental claims. The Employee Benefit Plan consists of  the attached  four (4) documents:
1. Employee Benefit Trust, Summary Plan Description, and Plan Document;
 
2. Employee Benefit Trust- Mexico Coverage, Summary Plan Description, and Plan Document;
 
3. Employee Benefit Trust Dental Benefit Plan, Plan Document Summary Plan Description and
 
4. Employee Benefit Trust Dental Benefit Plan-Mexico Coverage, Plan Document and Summary Plan Description.
Staff recommends approval of the final version of the plan prepared by EBSO.  

RECOMMENDATION / SUGGESTED MOTION:

I MOVE TO APPROVE AND RATIFY THE EMPLOYEE BENEFIT PLAN EFFECTIVE JULY 1, 2018 AS PRESENTED.
