Adopt Resolution #16-01-018 Approving Change Order #1 and Authorizing Final Payment to Douglas-Kerr Underground for
IP 14-24; Center Street
IP 14-24; Center Street
Purpose/Background:
Change Order #1 provided for the insulation of watermain that does not have sufficient soil cover to prevent freezing and for a new drainage structure and pipe section to allow for the connection of storm sewer pipes of different sizes and materials. Both items were identified during construction and were not detailed in the plans as bid.

Observations/Alternatives:
Issuing final payment will be contingent on receipt of the following items from Douglas-Kerr Underground LLC:
- IC 134 (Contractor’s Withholding Affidavit showing compliance with the provisions of Minnesota Statute 290.92 requiring withholding state income tax)
- Waiver of Mechanic’s Lien Rights (Affidavits that all claims against Douglas-Kerr Underground LLC by reasons of the contract have been fully paid or satisfactorily secured)
- Consent of Surety to Final Payment (Certification from the contractor’s surety)
Recommendation:
The Senior Engineering Technician has inspected the completed work and recommends final payment to Douglas-Kerr Underground LLC for IP 14-24;Center Street Improvements - Sunwood Drive to Ramsey Parkway in the amount of $33,335.45.
Action:
Motion to adopt Resolution #16-01-018 authorizing final payment to Douglas-Kerr Underground LLC for IP 14-24;Center Street Improvements - Sunwood Drive to Ramsey Parkway in the amount of $33,335.45.
