- Symetra Amendment No. 4, Voluntary Short Term Disability Income Insurance

Background
These policy documents are regarding the Symetra Employer sponsored Group Life Insurance and Group Voluntary Life, Short Term Disability/Long Term Disability insurance certificates and amendments as attached.
Three documents require signature:
1. Symetra Amendment No.3 Voluntary Long Term Disability Income Insurance
2. Symetra Amendment No. 4 Voluntary Short Term Disability Income Insurance
3. Symetra Group Life Certificate Rider
Two changes have been applied, one on April 1, 2023, as the waiting period changed from sixty (60) days to thirty (30) days and the second one, effective January 1, 2024, will apply as the Employer Group Life increased from $20k to $50k, and Employer Spouse Life increased from $5k to $10k.
Three documents require signature:
1. Symetra Amendment No.3 Voluntary Long Term Disability Income Insurance
2. Symetra Amendment No. 4 Voluntary Short Term Disability Income Insurance
