Reason for Recommended Action/Background
On May 8, 2018, the Board of Supervisors approved an agreement with Department of Health Care Services (DHCS) to receive funding in the amount of $13,943,284 for the Drug Medi-Cal Organized Delivery System.
Yolo County completed a formal competitive selection process (see Competitive Selection details below) and CommuniCare Healthcare Centers, Inc. was selected to provide Intensive Outpatient, Outpatient Services, Case Management, Physician Consultation, Recovery Services, and In-Person Screening/Referral Services.
On July 10, 2018, the Yolo County Board of Supervisors (“BOS”) approved the underlying Agreement (“Agreement 18-154”) with CommuniCare Healthcare Centers, Inc. for the provision of those services.
The Agreement has since been amended to shift funds, clarify language, and update rates.
On June 23, 2020 the BOS approved the third amendment reducing the total maximum compensation.
On July 27, 2021 the BOS approved the fifth amendment reducing (FY) 2020-21 funding and increasing (FY) 2021-22 funding.
On June 8, 2022the BOS approved the eight amendment that increased funding for (FY) 2021-22 and for (FY)2022-23.
Staff recommends approval of this eleventh amendment that increases funding for (FY) 2022-23 so CommuniCare Healthcare Centers, Inc. may continue providing these vital services to eligible Yolo County consumers. We are adding funding to four scopes of services to ensure that services continue and consumers continue to benefit from these services for the remainder of the fiscal year.
Performance Measures included in this Agreement are as follows:

HHSA confirms CommuniCare Healthcare Centers, Inc. has performed satisfactorily under this agreement 98.5% of consumers were retained in outpatient services. 34% reduction rate in substance use. 98% reduction in incarceration days.

Competitive Bid Process/Vendor Performance
On February 26, 2018, HHSA issued a Request for Proposal (“RFP”) seeking contractors to provide Substance Use Disorder (“SUD”) Adult Continuum of Care services to eligible Yolo County residents.
HHSA received eight proposals from this solicitation. Below is a table of all providers who submitted proposals. All proposals were evaluated, and the following proposals were determined to meet all the required qualifications necessary to successfully provide these services: CommuniCare, Turning Point, Progress House, Community Recovery Resources, Yolo Wayfarer Center, BayMark, and CORE. Because these proposals were considered to be of high quality, they will be offered a contract to provide these identified services. Separate agreements for the other bids chosen will be submitted to the BOS for approval. Heritage Oaks was the only bid received that will not be awarded a contract.
