Authorize the Yolo County Procurement Manager to execute any amendments necessary to extend agreements up to 90 days with California Forensic Medical Group for the provision of comprehensive medical services, professional behavioral health services, and jail-based competency treatment services to individuals in Yolo County detention facilities. Recommended Action
Authorize the Yolo County Procurement Manager to execute any amendments necessary to extend agreements with California Forensic Medical Group for the provision of comprehensive medical services, professional behavioral health services, and jail-based competency treatment services to the detainees of the Yolo County as follows:
- Authorize the Yolo County Procurement Manager to execute any amendments necessary to Agreement No. 16-194 with California Forensic Medical Group to extend the term of the Agreement through September 30, 2023 and increase funding in an amount not to exceed $299,352.45 by adding up to $289,949.64 of County General Funds and up to $9,402.81 of Medication Assisted Treatment (MAT) funds for 2023-24 for new contract maximum not to exceed $23,726,163.36, for the provision of comprehensive medical services to Yolo County detainees for the term of December 1, 2016 through September 30, 2023.
- Authorize the Yolo County Procurement Manager to execute any amendments necessary to Agreement No. 16-195 with California Forensic Medical Group to extend the term of the Agreement through September 30, 2023 and increase funding in an amount not to exceed $65,280.44 by adding up to $56,342.88 of County General Funds and up to $8,937.56 of Medication Assisted Treatment (MAT) funds for 2023-24 for new contract maximum not to exceed $3,952,910.97, for the provision of professional behavioral health services to Yolo County detainees for the term of December 1, 2016 through September 30, 2023.
- Authorize the Yolo County Procurement Manager to execute any amendments necessary to Agreement No. 22-172 with California Forensic Medical Group to extend the term of the Agreement through September 30, 2023 and increase funding in an amount not to exceed $66,269.33 by adding up to $66,269.33 of JBCT funds for 2023-24 for new contract maximum not to exceed $1,115,971.88, for the provision of jail-based competency treatment services to Yolo County detainees for the term of March 1, 2022 through September 30, 2023.

Reason for Recommended Action/Background
As explained in more detail below, HHSA and other County departments are currently negotiating new long-term agreements with CFMG for medical, behavioral health, and related services in Yolo County detention facilities. The agreements are complex and require careful consideration of the range of services to be provided, appropriate staffing levels, and related costs. The staff negotiating team will work diligently to present long-term agreements for Board consideration as soon as possible, but it is presently necessary to extend the existing contracts with CFMG during the negotiating period to avoid any interruption in services.
Approval of the recommended actions will enable the negotiating team to conclude short-term amendments in a form acceptable to the County Counsel. Without any extension, the current agreements will expire on June 30, 2023.
Medical Services - Agreement No. 16-194
Yolo County is mandated to provide effective and quality comprehensive medical services to detainees of the Yolo County Jail and Juvenile Detention Facility (Yolo Jail Facilities). On or about December 3, 2015, the Health and Human Services Agency (HHSA) in collaboration with the County Administrator’s Office (CAO), the County Probation Department (Probation) and the Sheriff’s Office (Sheriff), issued a request for proposal soliciting a provider of comprehensive medical services for the detainees in the Yolo Jail Facilities (“2015 Jail Medical RFP”).
Ultimately, the resulting agreement was awarded to California Forensic Medical Group, Inc. (CFMG) and thus on or about November 22, 2016, the County and CFMG entered into Agreement No. 16-194 (“Medical Services- Agreement No. 16-194”) for the provision of comprehensive medical services at the Yolo Jail Facilities, effective December 1, 2016.
On or about April 12, 2022 the County and CFMG amended the Agreement to add Medication Assisted Treatment (MAT) Services. MAT services include the use of FDA-approved medications, in combination with counseling and behavioral therapies, to treat substance use disorders as a medical disorder. The MAT program provides addiction treatment and recovery within correctional facilities. MAT also provides opioid addiction and other substance use disorders in more than 70 correctional facilities.
HHSA in collaboration with the CAO, Probation, and Sheriff now request authority to extend Medical Services- Agreement No. 16-194 for three months (or a portion thereof) not to exceed a maximum term extension through September 30, 2023, adding funding in an amount not to exceed $299,352.45 for fiscal year (FY) 2023-24 and delegating signing authority to the Yolo County Procurement Manager to assist with expediting execution of the extension to reduce the risk of a contract lapse and ensure continued compliance with federal and state regulations.
A formal solicitation via a request for proposal was released in FY 2022-23. The extension of Medical Services- Agreement No. 16-194 contemplated herein will allow time for the final negotiations to be completed and a new agreement to be awarded.
HHSA, CAO, Probation, and Sheriff believes it is the best interest of the County to extend this Agreement based on satisfactory service and to avoid the interruption of County business.
Behavioral Health Services - Agreement No. 16-195

Jail-Based Competency Treatment (JBCT) Services- Agreement No. 22-172
Over the past several years there has been a steady increase in the amount of felony incompetent to stand trial clients being referred to the Department of State Hospital for competency restoration without adequate capacity in the existing State Hospitals. This has led to a significant backlog of clients throughout the State waiting in jails for months at a time until placement can be made at a State Hospital bed. As a result, the Department of State Hospitals has been working with the State and various counties to increase statewide capacity for local treatment options. These options include Jail Based Competency Treatment programs. Yolo County is entered into an agreement with the State of California, Department of State Hospitals (“DSH”) for administration of a JBCT program. The purpose of the JBCT program is for restoration of competency to certain felony patient inmates deemed incompetent to stand trial (“IST”) under Penal Code section 1370 to occur locally in the jail rather than in a State Hospital for those determined to be appropriate.
Subsequently, County entered into Agreement No. 22-172 with California Forensic Medical Group for the County for the provision of JBCT services. CFMG was and remains the current jail medical services and behavioral health services provider for the County. CFMG will provides JBCT services to seven beds at the Monroe Facility. The services shall be provided 24 hours per day, seven days per week, including all state holidays.
HHSA, CAO, Probation, and Sheriff believes it is the best interest of the County to extend this Agreement based on satisfactory service and to avoid the interruption of County business.
