Recommended Action

Reason for Recommended Action/Background
Dignity Health, Sutter Health and Yolo County Health & Human Services Agency (HHSA) collaborated to develop and implement a Mobile Medicine Program. The goal was to provide appropriate services and linkages to additional community-based behavioral health, physical health, and dental health services. The Mobile Medicine Program aims to decrease severity and chronicity of otherwise manageable healthcare conditions for typically unserved or underserved individuals throughout Yolo County by providing preventive screenings, wound care, mental health services, dental care, health education, referral to social services and primary care follow up.
The initial 24-month period has now passed, and CommuniCare has created a fiscally sustainable model of care to continue these services. Funding sources that can be used include, but are not limited to, Medi-Cal billing for behavioral health, physical health and dental. Now, the County has received Housing and Homelessness Incentive Program (HHIP) funding from the Department for Healthcare services. HHIP aims to improve health outcomes and access to whole person care services by addressing housing insecurity and instability as social determinants of health for the Medi-Cal population.
Performance Measures included in this Agreement are as follows:

HHSA confirms CommuniCare has performed satisfactorily under this agreement. This program served 313 unique patients in the first half of 2023 (1/1/23-6/30/23). Mobile Medicine provided 515 primary care visits, and 75 behavioral health visits to Yolo County’s most vulnerable populations in this period.

Competitive Bid Process/Vendor Performance
On March 10, 2021, the County issued a Request for Proposals (RFP) from qualified community-based organizations to provide Mobile Medicine Services. The RFP was open for six (6) weeks, and staff offered potential bidders an opportunity to ask questions during a Mandatory Bidders’ Conference on March 19, 2021. The County received one proposal from this solicitation from CommuniCare. CommuniCare’s proposal was evaluated by the RFP Review Committee and was determined to meet all the required qualifications necessary to successfully provide these services. HHSA confirms CommuniCare Health Centers has performed satisfactorily under this agreement. Thus, pursuant to the authority provided to the County Procurement Manager as the County Purchasing Agent under the current Yolo County Procurement Policy, the Procurement Manager has determined that it is in the best interest of the County to extend this Agreement, based on satisfactory service and to avoid the interruption of County business.
Additionally, the County and CommuniCare will submit a proposal regarding the Mobile Medicine Program to the Yolo County Executive Commission to Address Homelessness (“the Commission”) for review and approval. The Commission is a board established to have one overarching planning body that develops and oversees a community-wide vision for ending homelessness. The County and community service providers present proposals to the Commission for review and approval. Members of the Commission are appointed by the jurisdictional or organizational governing body representing each position on the Commission. The Commission consists of six (6) Voting Members: one (1) individual who serves as a currently elected official in the City of Woodland, one (1) individual who serves as a currently elected official in the City of West Sacramento, one (1) individual who serves as a currently elected official in the City of Davis, one (1) individual who serves as a currently elected official in the City of Winters, one (1) individual who serves as an elected County Supervisor, and one (1) individual who serves as the elected representative of the Homeless Continuum of Care. Pursuant to the Commission’s Bylaws, one of their role(s) is to ensure the implementation of goals and strategies that address and achieve functional homelessness in the County.

No general funds are required by this action. These services will be funded by HHIP. This action increases the contract maximum by $526,223.63, for a new total of $1,137,277.13, for the period of September 1, 2021 through August 31, 2024. The amount of $526,223.63 is included in the HHSA adopted budget for fiscal year 2023-24. The related funding will be included in the requested budget process for future fiscal years.
