Reason for Recommended Action/Background
California Advancing and Innovating Medi-Cal (CalAIM) is a series of initiatives and reforms to improve Medi-Cal quality and equity. PATH (Providing Access and Transforming Healthcare) is a component that encompasses several initiatives that includes providing funding to support the statewide CalAIM justice-involved initiatives. One of these initiatives provides funding to support youth reentering their communities following in custody commitments. This initiative seeks to guarantee that incarcerated youth have an individualized reentry plan no matter the length of their stay to include healthcare support, appropriate treatment service referrals, education and vocational training,and housing assistance.
Probation submitted an Implementation Plan as a collaborative effort with HHSA and ITSD to comply with the requirement of CalAIM PATH for youth reentering the community after commitments of various duration at the Juvenile Detention Facility. Probation was awarded $2.5 million in funding for FY’s 24-25 and 25-26. This funding is being utilized to pay for managers and staff as they develop policies and procedures to ensure that each youth leaves with a comprehensive reentry plan and community referrals that will meet their long-term needs. Interagency cooperation is imperative to the success of this initiative and the team members from each department have brought their commitment and ingenuity to this process and is moving toward a full implementation date of July 1, 2026.

In October 2024, Probation submitted the Yolo County CalAIM Implementation Plan, which was approved, and Probation was awarded $2.5 million to provide funding for the implementation over a two-year period (July 1, 2024, to June 30, 2026). In FY 2024-25, funding of $1.322 million is already included in the adopted budget. The funding is provided to Probation, HHSA and ITSD as follows:

*Probation includes budget for community-based provider agreements such as the provision of healthcare to the CalAIM population and training for implementation of CalAIM initiatives.
CalAIM has initially been funded by the State as a five-year project. It is unclear currently if long-term funding is available to support medical and behavioral health expenses in the long term.
