Approve fourth amendment to Agreement No. 23-152 with OLE Health dba CommuniCare OLE to reduce funding in the amount of $56,250 for fiscal year 2025-26 for a new contract maximum of $888,750 for the provision of Woodland Community College behavioral, physical and social services.

Reason for Recommended Action/Background

In January 2022, California Department of Health Care Services (DHCS) rolled out the CalAIM initiative aimed to provide broad delivery system program and payment reform across the Medi-Cal system. The goal was transformation of the Medi-Cal delivery system, moving it towards a population health approach that prioritizes prevention and whole person care. Focus shifted from compliance to quality and performance metrics. Specialty Mental Health Services (SMHS) requirements and policies pursuant to the CalAIM initiative were updated, and as a result of these updates, the County was required to revise the terms and conditions in the provider agreements for these services.

On or about June 27, 2023, the County and CommuniCare Health Centers entered into a one-year renewal agreement for the provision of Woodland Community College behavioral, physical and social services in the amount of $315,000 for the period of July 1, 2023 through June 30, 2024.

On or about October 24, 2023, the County and CommuniCare Health Centers entered into a Consent to Assignment and Novation Agreement, effective October 1, 2023, with OLE Health dba CommuniCare OLE.

On or about May 21, 2024, the Parties amended the Agreement to extend the term through June 30, 2025 and added funding for FY2024-25.

On or about January 21, 2025, the Parties further amended the Agreement to update County email addresses and added rates for FY2024-25.

On or about May 1, 2025, the County exercised its option to extend the term of the Agreement through FY2025-26.

On or about October 4, 2025, the Parties further amended the Agreement to add rates for FY2025-26.

HHSA is requesting approval of this fourth amendment to reduce funding in the amount of $56,250 for FY2025-26 to align with the Board of Supervisor's decision to reduce MHSA funding.  The funding reductions will result in fewer students having access to these services. However, no individuals that are presently accessing services will be impacted.

Competitive Bid Process/Vendor Performance

On September 24, 2020, the County issued a competitive solicitation via a request for proposals (RFP) from qualified community-based healthcare providers to provide physical healthcare, behavioral healthcare and related social services to college students at the Woodland Community College. The RFP was open for 41 days, just under 6 weeks, and staff offered potential bidders an opportunity to ask questions during a Mandatory Bidders’ Conference on October 7, 2020.  Bidders were offered the opportunity to submit proposals by dropping off a paper proposal directly at HHSA.

The County received one proposal(s) from this solicitation.  The proposal submitted by CommuniCare Health Centers was evaluated and determined to meet all the required qualifications necessary to successfully provide these services. Additional information regarding the Competitive Bid Process is provided below, including a summary of the RFP evaluation criteria.

Performance Measures
HHSA confirms CommuniCare OLE is performing satisfactorily under this Agreement. Performance Measures were originally included in the underlying Agreement and are not being revised at this time (See Att. B. Performance Measures). Based on the last two quarters of performance measures submitted, the majority of students receiving services were located at the Woodland Community College Campus. In the first quarter of the current fiscal year, the students who completed surveys reported 100% satisfaction with the timeliness of their initial appointment and 100% reported satisfaction with the access and services provided.

No general funds are required for this action. These services are funded by Mental Health Services Act (MHSA) and funds provided by the Yuba Community College District (YCCD). The total reduction amount of this action is $56,250 for the period of July 1, 2023 through June 30, 2026. The reduced amount of $56,250 will be adjusted in the FY2025-26 budget per Board of Supervisor's direction in October 2025..
