Adopt budget resolution amending the Health and Human Services Agency budget for Fiscal Year 2025-26 to reflect an increase of $317,921 to the Adult & Aging Homeless Services cost center; and approve agreement with Yolo Community Care Continuum – Farm House in the amount of $317,921 for the period of October 1, 2025 through March 31, 2029, for the provision of operating subsidy payments. Recommended Action

  1. Adopt budget resolution amending the Health and Human Services Agency budget for Fiscal Year 2025-26 to reflect an increase of $317,921 to the Adult & Aging Homeless Services cost center. (See Att. A. Budget Resolution); and
     
  2. Approve agreement with Yolo Community Care Continuum in the amount of $317,921 for the period of October 1, 2025 through March 31, 2029, for the provision of operating subsidy payments. (See Att. B. Agreement).

Reason for Recommended Action/Background

Budget Resolution
On September 23, 2025, the Board of Supervisors for the County of Yolo adopted a budget for fiscal year 2025-26 for the Health and Human Services Agency (“HHSA”). During the adopted budget development process, HHSA fiscal staff were unaware of the need to include Community Care Expansion (“CCE”) Preservation Program/Operating Subsidy Payment (“OSP”) funding for fiscal year 2025-26; therefore, the funds were not included in the adopted budget process.

For reference, on or about August 15, 2022, the California Department of Social Services (“CDSS”) notified HHSA of its CCE Preservation Program award allocation. On or about April 7, 2023, CDSS notified HHSA of its final CCE award amount in two categories: Operating Subsidy Payment funds and Capital Project funds. Thereafter, on or about June 26, 2023, HHSA entered into an agreement with Advocates for Human Potential, Inc., CDSS’ third-party administrator of the CCE Preservation Program (“AHP Agreement”). The AHP Agreement included requirements on how HHSA can use its CCE allocation.

Approval of the request herein will enable HHSA to add the amount of $317,921 to the Health and Human Services Agency adopted budget for fiscal year 2025-26 and subsequently enter into a contract with Yolo Community Care Continuum funded with the CCE-OSP funds.

Agreement
Yolo Community Care Continuum (YCCC) is the owner of the adult residential facility Farmhouse located in Davis, California. Farmhouse is licensed as adult residential facilities by California Department of Social Services (CDSS) serving Yolo County clients diagnosed with serious mental illness. It provides housing and services to 10 adults at the facility. This adult residential facility is the only long-term residential treatment home in Yolo County for people with serious mental illness including those who are experiencing or at risk of homelessness.

YCCC the owner of Farmhouse adult residential facilities (ARF), is one of three facility types that are eligible for funding under the community care expansion grant. Yolo Community Care Continuum is the only facility which applied to utilize the funding for Community Care Expansion Operating Subsidy grant funds. The grant requires a facility to be one of the following types: Adult Residential Facilities (ARF), Residential Care Facilities for the Elderly (RCFE), or Residential Care Facility for the Chronically Ill (RCFCI). The facility must also be currently licensed, be in good standing with Community Care Licensing Division (CCDL) and serve at least one qualified resident. Yolo Community Care Continuum meets the requirements.

Competitive Bid Process/Vendor Performance

On October 22, 2025, the County’s Deputy Director of GSD/Manager of Procurement/County Purchasing Agent (Purchasing Agent) approved a sole/single source procurement. Without a sole/single source approval for these services, the County would be negatively impacted as HHSA would have to release the funding back to the State to redistribute to other counties and HHSA could possibly lose beds at Farmhouse. Factors included but were not limited to: Yolo Community Care Continuum is the owner of Farmhouse Adult Residential Facility (ARF) which is one of the three facility types that are eligible for this funding. Yolo Community Care Continuum is the only facility that applied to utilize this funding, no other local facilities that are eligible for the funding responded to the application solicitating interest which was required under the grant.

Vendor Performance
This Agreement does not include associated performance measures. There are currently no Performance Outcomes or Results-Based Accountability (RBA) metrics for this agreement because it represents a new scope of service. HHSA also does not have comparable RBAs from prior agreements of similar scope, as this is a new program area.

The purpose of this Agreement is to provide Operating Subsidy Payments (OSP) to eligible adult and senior residential care facilities that serve applicants and recipients of Supplemental Security Income/State Supplementary Payment (SSI/SSP) and the Cash Assistance Program for Immigrants (CAPI). These payments are directed to facilities, not to individual clients, and therefore do not involve direct client services.

No general funds are required for this action. The action will allow the forthcoming expenditure contract to have an amount of $317,921 for fiscal year 2025-26 (pending approval of the attached budget resolution). These services will be funded by Community Care Expansion (CCE) grant allocation. The amount of $317,921 will be added to the HHSA adopted budget for fiscal year 2025-26 (pending approval of the attached budget resolution). Any related funding will be included in the appropriate budget process for future fiscal years.
