Recommended Action
- Approve fourth amendment to Agreement No.19-144 with Yolo Community Care Continuum (YCCC) to extend the term of the agreement through June 30, 2023 and add funding in the amount of $524,200 by adding $28,500 for Fiscal Year (FY) 2021-22 and add funding in the amount of $495,700 for FY 2022-23 for a new contract maximum of $1,987,291 for the period of July 1, 2019 through June 30, 2023; and to update and clarify language in the agreement, for the provisions of adult residential services;
- Re-Delegate authority to the Director of the Yolo County Health and Human Services Agency or designee to: 1) exercise optional year extensions up to $495,700 per fiscal year for fiscal year 2023-24 and 2) issue related option year notices. Please see Section 2. of Att. A. Fourth Amendment; and
- Delegate authority to the Director of the Yolo County Health and Human Services Agency or designee to execute amendments to increase funding for Fiscal Year 2021-22 by an amount not to exceed $50,000 in order to pay final claims as needed.

Reason for Recommended Action/Background
Yolo County serves Seriously Mentally Ill adults meeting the mental health medical necessity criteria that require the long-term residential treatment services provided by the YCCC Farmhouse Program. This program helps clients, while living onsite, increase their ability to live independently through skill building, job training and/or other educational programs. This will further client independence and potentially reduce costs to the County by reducing psychiatric hospital stays, Institute for Mental Disease (IMD) placements, and homelessness.
Approval of this fourth amendment adds $28,500 to fiscal year 2021-22, so pending claims can be paid and revises and clarifies contract language. These additional funds are needed as HHSA placed additional clients, served in our Forensic Full Service Partnership program, at Farmhouse this fiscal year.
Performance Measures were included in Agreement No. 19-144 and are not being changed by this amendment.

No general funds are required by this action. These services will be funded by MHSA. The action increases the contract maximum by $524,200 by adding $28,500 for fiscal year 2021-22 and adds funding in the amount of $495,700 for fiscal year 2022-23 for a total of $1,987,291 for the period of July 1, 2019 through June 30, 2023. The amount of $28,500 is included in the HHSA adopted budget for FY2021-22. The amount of $495,700 was included in the HHSA proposed budget for FY2022-23.

In no event shall the term of the Agreement extend beyond June 30, 2024 nor shall the total contract maximum exceed the amount of TWO MILLION FOUR HUNDRED EIGHTY-TWO THOUSAND NINE HUNDRED NINETY-ONE DOLLARS ($2,482,991) unless otherwise agreed to in writing by the parties and in conformity with the then-current Yolo County Procurement Policy approved by the Yolo County Board of Supervisors.
