Recommended Action
- Approve agreement with Yolo Community Care Continuum in an amount of $2,250,000 for the period of July 1, 2024 through June 30, 2027 for the provisions of Adult Residential Services through the Farmhouse Program in compliance with the new California Advancing and Innovating Medi-Cal (CalAIM) program; and
- Delegate authority to the Director of the Yolo County Health and Human Services Agency or her/his designee to: 1) exercise optional year extensions up to $750,000 per fiscal year for fiscal years 2027-28 and 2028-29 and 2) issue related option year notices. Please see Section III of Att. A. Agreement

Reason for Recommended Action/Background
The Farmhouse Residential Treatment Program is a therapeutic residential setting that provides services 24 hours a day, 7 days a week with a full range of social rehabilitation services for individuals diagnosed with a severe mental illness (SMI) who require intensive support in order to avoid hospitalization or institutionalization.
Farmhouse is a DHCS licensed Adult Residential Facility and the only long-term residential treatment program available for (SMI adults in Yolo County that provides a full range of social rehabilitation services including:
Farmhouse is a DHCS licensed Adult Residential Facility and the only long-term residential treatment program available for (SMI adults in Yolo County that provides a full range of social rehabilitation services including:
- Individual group counseling
- Crisis intervention
- Planned activities
- Counseling with family, as applicable
- Development of Community support systems for clients
- Transportation to necessary medical/community appointments
- Client advocacy
- Socialization activities in the community
- Pre-vocational and vocation services
- Program length and date of discharge based on client’s needs.
HHSA request approval of this agreement with Yolo Community Care Continuum, for the provision of adult residential services. Approval of this agreement will continue enabling County to meet the needs of its SMI clients that require intensive support in order to avoid long term hospitalization or institutionalization.

No general funds are required by this action. These services will be funded by a combination of funding. The total amount of this action is $2,250,000 for the period of July 1, 2024 through June 30, 2027. The amount of $750,000 is included in the HHSA recommended budget for FY 2024-25. The related funding will be included in the requested budget process for future fiscal years.

In no event shall the term of the Agreement extend beyond June 30, 2029, nor shall the total contract maximum exceed the amount of THREE MILLION SEVEN HUNDRED FIFTY THOUSAND DOLLARS ($3,750,000), unless otherwise agreed to in writing by the Parties and in conformity with the then-current Yolo County Procurement Policy approved by the Yolo County Board of Supervisors.
