Recommended Action
- Accept the California Providing Access and Transforming Health (PATH) grant award in the amount of $58,980 for expansion of Yolo 211 Medi-Cal referral services (Att. B);
- Approve agreement with Community Link Capital Region for Yolo 211 services, including expansion of 2-1-1 Medi-Cal referral services, from July 1, 2024 through June 30, 2025, not exceed $250,000 per fiscal year (Att.C); and
- Adopt a Budget Resolution in the amount of $152,332 for Yolo 211 Program Expansion (Att.D.)

Reason for Recommended Action/Background
The County applied for and was awarded the PATH (round 3) grant in the amount of $58,980 toward expanded Yolo 211 Medi-Cal referral services. The 211 code has been used by the County for more than 10 years with the operation of a regional 211 service known as Yolo 211 or 2-1-1 Yolo. Yolo 211 connects residents via 24/7 call center or online resource directory to resources in the County. Yolo 211 is operated by Community Link. The 211 center’s referral specialists receive requests from callers, access databases of resources, match the callers’ needs to available resources, and link or refer them directly to an agency or organization that can help. Resource topics included but are not limited to local health and human service programs, unemployment support, basic needs programs, aging-related programs, mental health, youth support and non-emergency disaster response assistance.
Staff is requesting that the Board approve and authorize the Board Chair to sign the contract with Community Link Capital Region to continue and extend Yolo 211 service for appropriate referrals for individuals seeking Medi-Cal assistance, affordable insurance options, and connections to services under the Medi-Cal expansion through CalAIM. This amendment allows the County to exercise two additional option years for service extension.
