Recommended Action

Approve third amendment to Agreement No. 23-317 with C.O.R.E. Medical Clinic to extend term of the Agreement through June 30, 2026 and add funding in the amount of $2,400,000, by increasing 2024-25 by $600,000 and adding $1,800,000 for 2025-26 for a new contract maximum of $4,347,030.16, for the period of July 1, 2024 through June 30, 2026 and to update scope of work to include narcotic treatment services for youth under the age of 18, for the provision of Substance Use Disorder Continuum of Care services.

Reason for Recommended Action/Background

The County entered into an agreement with the California Department of Health Care Services (DHCS) to provide Drug Medi-Cal Organized Delivery System (DMC-ODS) substance use disorder (SUD) treatment services to eligible clients. DMC-ODS is a program for organized SUD services to Medi-Cal eligible individuals with SUD residing in a county that has elected to participate in the DMC-ODS. The program provides members with access to care and system interaction needed to achieve sustainable recovery. The County also receives funding from a variety of other federal and state funding streams that assist the County in providing SUD treatment services to various client populations, including but not limited to: Substance Abuse Prevention and Treatment (SABG) Block Grant Funds; Substance Abuse and Mental Health Services Administration (SAMHSA); CalWORKs and CWS funding; 2011 Realignment, Opioid Settlement Funds, and Intergovernmental Transfer (IGT) Funds.
 
The DMC-ODS/SUD Continuum of Cares services includes the following:
· Residential treatment services
· Outpatient and intensive outpatient treatment services
· Residential withdrawal management (detoxification) services
· Care Coordination services
· Recovery services
· Clinician Consultation
· Recovery Residence
· Narcotic treatment program services/opioid treatment program

These services have been provided to eligible Yolo County resident referred through an access point established by Yolo County or referred by provider and have included, but not be limited to, the following populations/programs:
· CalWORKS Participants
· Individuals with an open Child Welfare Services (CWS) Case Plan
· Individuals who are Pregnant or Postpartum (Perinatal Services)
· Individuals who are involved with the Criminal Justice and those partially funded by system (AB109)
· Medically Indigent Adults
· Individuals with Co-Occurring substance use and mental health disorders and SUD disorders.
 
After a competitive solicitation process (details below), the County entered into Agreement No. 23-317 with C.O.R.E. Medical Clinic, Inc (C.O.R.E.) to provide Opioid/Narcotic Treatment Program (OTP/NTP) services in Narcotic Treatment Program licensed facilities, Care Coordination, and Clinician Consultation Services. The services that C.O.R.E. provides are crucial to helping Yolo County meet the needs of our residents who struggle with substance use and need OTP/NTP services as part of their continuum of care.

Approval of this third amendment will allow for additional funding for 2024-25, extend services through June 30, 2026, add funding for 2025-26 and update the scope of work to include referred client through the Probation Department and allow for services to youth under the age of 18.  Based on current utilization, it is anticipated that contract funds for the current fiscal will be fully expended by March 2025. The additional funding will carry the contract through the remainder of the fiscal year.

Competitive Bid Process/Vendor Performance

On March 3, 2023, the County via the Department of General Services, Procurement Division (DGS-Procurement) issued a Request for Proposal (RFP) on behalf of the Health and Human Services Agency (HHSA) for Substance Use Disorder Adult Continuum of Care services. The solicitation was open for 64 days, (just over 9 weeks). On March 24, 2023, the County offered potential bidders an opportunity to ask questions during a bidder’s conference.  The County received three (3) proposal(s) from this solicitation.  DGS reviewed the proposal for minimum requirements and all three (3) proposals were moved forward to the Review Committee, comprised of two (2) HHSA staff members, one (1) Yolo County Probation staff member, and one (1) Placer County staff member. The Review Committee evaluated the proposals and determined that the proposal submitted by C.O.R.E. Medical Clinic, Inc., met all the required qualifications necessary to successfully provide these services.  On or about June 27, 2023, the County issued an Intent to Award letter.  Additional information regarding the Competitive Bid Process is provided below, including a summary of the solicitation evaluation criteria.

Performance Measures
HHSA confirms C.O.R.E. Medical Clinic is performing satisfactorily under this Agreement. Performance Measures were originally included in this Agreement. Performance Measures are not being revised at this time (See Att. B. Performance Measures). One hundred percent (100%) of clients referred received a service within 14 days of diagnosis and two (2) or more services at 30 days and 73% of referrals reported a decrease in substance use at completion of treatment.

No general funds are required by this action. These services will be funded by SUD Realignment and Opioid Settlement. The action increases the contract maximum by $2,400,000 by adding $600,000 for 2024-25 and $1,800,000 for 2025-26 for a total of $4,347,030.16 for the period of July 1, 2024 through June 30, 2026. The amount of $600,000 is included in the HHSA adopted budget fiscal year 2024-25. The amount of $1,800,000 will be included in the 2025-26 budget process. The related funding will be included in the requested budget process for future fiscal years.

This agreement already includes two (2) one-year optional extensions and one (1) 6-month optional extension for Fiscal Years 2026-27 and 2027-28, plus July 1, 2028 through December 31, 2028 in an amount less than or equal to $1,080,000 per fiscal year. The amounts for these future years are not being increased by this third amendment.
