Receive an update on a Board Directed Projects related to the Behavioral Health Crisis Receiving Center that includes options for proceeding or terminating the project; and consider approval of staff recommendation to proceed with the project by authorizing the Yolo County Health and Human Services Agency (HHSA) to enter into a contract with WellSpace Health for three (3) crisis chairs (in a behavioral health receiving center located in Sacramento) that will serve all of Yolo County. Recommended Action

  1. Receive an update on a Board Directed Project related to the Behavioral Health Crisis Receiving Center that includes options for proceeding or terminating the project; and
     
  2. Consider approval of staff recommendation to proceed with the project by authorizing the Yolo County Health and Human Services Agency (HHSA) to enter into a contract with WellSpace Health for three (3) crisis chairs (in a behavioral health receiving center located in Sacramento) that will serve all of Yolo County.

Reason for Recommended Action/Background

On April 29, 2025, the Yolo County Health and Human Services Agency (HHSA) informed the Board that increasing costs and inadequate funding were impeding the Agency’s ability to move forward with the development of a Behavioral Health Crisis Receiving Center (BHRC) that had been informally dubbed “Crisis 2.0.”  At that time, the Board was presented with several options, including the approved plan of exploring contracts with two providers (Yolo Continuum of Care Safe Harbor facility in Woodland and WellSpace Criss Receiving for Beahvioral Health "CRBH" facility in Sacramento) to develop a one-year pilot to provide services similar to what would have been provided in the Crisis 2.0 proposal.  The plan was for each facility to provide three (3) beds each, for a total of six (6) beds; this was closely aligned with the anticipated number of beds – eight (8) – that were proposed in Crisis 2.0.

HHSA worked closely with several local partners to commit funding for the one-year pilot including: the Cities of Davis, Woodland, and West Sacramento; Dignity Health; and Sutter Health. HHSA has also secured funding from a Board of State and Community Corrections (BSCC) grant and intends to utilize Board Directed Projects funding to cover any funding gaps.

HHSA was given permission from Procurement to pursue sole source contracts with the identified providers, but has determined that it is not possible to develop a contract for BHRC services with Yolo Continuum of Care because the proposed services would create a violation for the Safe Harbor facility with California Community Care Licensing (CCL). HHSA has developed a contract with WellSpace Health for the Board’s consideration that can provide three (3) beds in the CRBH facility to support the one-year pilot program.  Because of the limited number of beds, the location of the WellSpace facility, and the absence of another viable Yolo County option, HHSA is seeking guidance from the Board regarding whether this project should move forward. 

HHSA is providing the Board with three options to consider:

Option 1: Move forward with a one-year pilot that serves the entire County using the WellSpace Health CRBH facility.
  • Pros: Allows for community to finally have a place for patients in need of a receiving center.
  • Cons: The limited number of beds is inadequate to cover the need of the entire County.  The location of the facility will require intensive coordination for transportation for individuals served outside of West Sacramento.
Option 2: Do not execute the WellSpace contract and continue to seek another vendor for Davis & Woodland.
  • Pros: This approach could result in obtaining a vendor that better covers the rest of the County’s geographic needs.
  • Cons: This would further delay implementation, as there is no known vendor that exists that can affordably provide these services within Yolo County.  This option could result in losing the WellSpace option and the ability to provide any level of service in the county. 
Option 3: Terminate the Board Directed Project for the Behavioral Health Crisis Receiving Center.
  • Pros: Allows the Board to redirect Board Directed Projects funding to address other financial needs.
  • Cons: This would eliminate any possibility of Yolo County having any level of a crisis receiving center.
HHSA is respectfully recommending that the Board approve Option 1, which would develop a one-year pilot project for a Behavioral Health Receiving Center that serves the entire county.  The intent of the project would be to develop processes and gather information to inform a future project that would expand upon the model to better meet the County’s needs. This contract, if approved, is a flat-rate of $500,000 per year to reserve the three (3) crisis chairs at the WellSpace Health Crisis Receiving for Behavioral Health (CRBH) program located in Sacramento, California.   This contract will not impact General Fund.  This approach will use only $175,000 of the $800,000 of Board Directed Funds that had been approved for the Crisis Receiving Center pilot, leaving $625,000 for future projects.

This recommendation for this action will have no impact on the County General Fund. The one-year pilot project will be paid for by the Byrne State Crisis Intervention Program. HHSA is respectfully recommending that the Board approve Option 1, which would develop a one-year pilot project for a Behavioral Health Receiving Center that serves the entire county. The intent of the project would be to develop processes and gather information to inform a future project that would expand upon the model to better meet the County’s needs. This project, if approved the project will consist of the following expenses:
