Recommended Action

Approve first amendment to Agreement No. 24-193 with the City of West Sacramento to reduce fiscal year (FY) 2024-25 by $221,243.85; increase FY2025-26 by $93,051.70; and increase FY2026-27 by $78,192.15 for an overall reduction of $50,000 and a new contract maximum of $2,647,200, for the period of July 1, 2024 through June 30, 2027 for the provision of homeless services via the Behavioral Health Bridge Housing program.

Reason for Recommended Action/Background

On or about June 23, 2023, the California Department of Health Care Services (“DHCS”) awarded funding to County to operate bridge housing settings to address the immediate and sustainable housing needs of people experiencing homelessness who have serious behavioral health conditions, including serious mental illness (“SMI”) and/or substance use disorder (“SUD”).

On or about October 3, 2023, County entered into an agreement with Advocates for Human Potential, Inc. to serve as the administrative entity supporting County’s implementation of the Behavioral Health Bridge Housing (“BHBH”) Program.

On or about June 25, 2024, County entered into an agreement with the City of West Sacramento (“City”) to provide these services. Under this agreement, City has provided temporary emergency/interim housing for homeless individuals with behavioral health needs and/or substance use disorders, including thirty-one (31) beds and two (2) local motel locations. The City has also provided the following services under this agreement:

· Low intensity case management services aided at stabilization, including development of a case plan to obtain permanent housing
· Housing navigation services
· Peer support services
· Employment assistance

Approval of this amendment reduces the overall funding for this agreement by $50,000 by reducing FY2024-25 in the amount of $221,243.85. This reduction aligns the funding to the adopted budget for FY2024-25.

Even though the overall agreement funding is being reduced for FY2024-25, fiscal years 2025-26 and 2026-27 are being increased by $93,051.70 and $78,192.15 respectively.

Competitive Bid Process/Vendor Performance

On or about April 30, 2024, the then acting Yolo County Procurement Manager or designee approved a sole source procurement for these services. A sole source was deemed appropriate for these services because the City of West Sacramento was one of only two (2) cities willing to participate in the BHBH Program and was committed to adding beds for individuals with a history of SUD or SMI within the timeframe and requirements set by the State. The BHBH grant obtained by County required that County add additional beds to its inventory within twelve (12) months.

Vendor Performance
HHSA confirms that the City of West Sacramento is performing satisfactorily under this Agreement. Performance Measures were originally included in this Agreement are not being revised at this time (See Att. B. Performance Measures).

The City of West Sacramento (“City”) provides homeless services via Behavioral Health Bridge Housing (“BHBH”) program. For Fiscal Year 2024-25 City of West Sacramento purchased and renovated the motels to house the BHBH clients. Doors at the W. Sacramento BHBH facility opened 10/1/2025 as scheduled in contract. From 10/1/2025 to 11/19/2025 the City of W. Sacramento has provided interim housing to 36 severe SMI and/or SUD homeless individuals and connected them to services.

No general funds are needed for this action. These services are funded by BHBH Grant funds. This action reduces funding for FY2024-25 in the amount of $221,243.85; increases funding for FY2025-26 by $93,051.70; and increases funding for FY2026-27 by $78,192.15 for a new contract maximum of $2,647,200 and a net decrease of $50,000.

The additional funding for FY 2025–26 is included in the adopted budget.
