Recommended Action
- Approve first amendment to Agreement No. 21-215 with CommuniCare Health Centers, Inc. to increase funding in the amount of $30,000 total, with $15,000 funding for each of November and December, for a new contract maximum of $58,455.72; update the term of the agreement to become effective November 1, 2021 through December 31, 2021; and add increased optional monthly extensions for the provisions of Project Room Key services; and
- Re-delegate authority to the Director of the Yolo County Health and Human Services Agency or her/his designee to 1) exercise optional monthly extensions up to $29,227.86 per month and 2) issue related optional notices. Please see Section III. of Att. A. First Amendment.

Reason for Recommended Action/Background
The California Department of Social Services (“CDSS”) released a Project Room Key (“PRK”) allocation of funding to continue to provide Project Room Key and Rehousing strategy for non-congregate shelter and rehousing services. CommuniCare has been providing primary support/coverage for individuals identified as eligible in the City of Davis to access a motel room for their quarantine/isolation period. CommuniCare has taken the lead as the Primary Motel Homeless Services point of contact. CommuniCare will provide adequate staff available to respond Monday through Friday 8:00 a.m. to 5:00 p.m. as well as on-call coverage from 8:00 a.m. to 8:00 p.m. on weekends and holidays. CommuniCare will respond to requests for immediate need such as securing motel rooms, meals, transportation, medication, and other supplies as determined to be required for client safety while determining eligibility for access to services.
Currently, this Agreement has an effective date of September 1, 2021, however, the expanded services under this new CommuniCare agreement will not begin until the Project Room Key program ends in the City of Davis. Currently, the Project Room Key program in Davis has been extended through October 31, 2021, and no services have been performed or will be performed under this agreement for the months of September and October 2021.
Approval of this amendment will allow CommuniCare Health Centers to continue these vital services to our vulnerable population and ensure COVID-positive clients are placed in a motel during quarantine.

Further explanation as needed:
No general funds are required by this action. These services will be funded by CDSS PRK/Rehousing. The action increases the contract maximum by $30,000 for a total of $58,455.72 for the period of November 1, 2021, through December 31, 2021. The $30,000 increase is made up of increases of $15,000 for November and December each. The amount of $30,000 is included in the HHSA adopted budget for FY2021-22. If all Optional Monthly extensions are taken (January through June 2022), the ongoing commitment would be $233,823.
The following is the breakdown of funding for this agreement.
The agreement also includes six (6) one-month optional extension through June 30, 2022, in an amount less than or equal to $29,227.86 per month.
In no event shall the term of the Agreement extend beyond June 30, 2022, nor shall the maximum payment obligation exceed the amount of TWO HUNDRED THIRTY-THREE THOUSAND EIGHT HUNDRED TWENTY-TWO DOLLARS AND EIGHTY-EIGHT CENTS ($233,822.88), unless otherwise agreed to in writing by the parties and in conformity with the then-current Yolo County Procurement Policy approved by the Yolo County Board of Supervisors.
