Approve agreement with Yolo Community Care Continuum (YCCC) in the amount of $100,000 for the period of July 1, 2022 through June 30, 2023 for the provision of permanent supportive housing services.

Reason for Recommended Action/Background

A full-time Social Services Coordinator will split their time between Homestead and Pacifico housing complexes, providing direct service, connecting residents with other services, facilitating support groups, helping evaluate residents and recommending appropriate placement. The overarching goal is to improve the general health and stability of residents through consistent daily interaction. This approach will help identify and solve problems before they escalate. The Social Services Coordinator will provide individual counseling and crisis intervention for residents, as needed.
