Recommended Action

Approve second amendment to Agreement No. 19-232 with Yolo County Children’s Alliance to increase funding for FY2022-23 in the amount of $70,000 for a new contract maximum of $1,393,860.51 for the period of November 1, 2019, through September 30, 2023 to provide and expand the parameters for implementing an existing Nurse-Family Partnership (NFP) or Healthy Families America (HFA) home visiting program; and to update and clarify language in the agreement.

Reason for Recommended Action/Background

The Patient Protection and Affordable Care Act of 2010 established the Maternal, Infant and Early Childhood Home Visiting (MIECHV) Program to provide an opportunity for collaboration and partnership at the federal, state, and community levels to improve outcomes for families who reside in at-risk communities through evidence-based home visiting programs. The California Home Visiting Program (CHVP) strives to develop collaborative community systems that protect and improve the health and developmental outcomes for California’s pregnant women, parents, and families. 

On or about November 5, 2019 the County entered into an agreement with Yolo County Children’s Alliance (YCCA) to provide and expand the parameters for implementing an existing Nurse-Family Partnership (NFP) or Healthy Families America (HFA) home visiting program in accordance with Federal MIECHV and State requirements to achieve positive outcomes for each of the following five goals:
  1. Provide leadership and coordinate maternal and early childhood systems and supports to advance federal, state, and local efforts to improve the health and well-being of families in California
  2. Cultivate strong communities
  3. Promote maternal health and well-being
  4. Improve infant and child health development
  5. Strengthen family functioning
This agreement has been extended via option letter extensions for FYs 2020-21, 2021-22, and 2022-23 which added $335,000 per fiscal year.

HHSA requests the approval of this amendment which adds an additional $70,000 of Public Health Realignment funds through the Health Equity Prevention Fund – Children’s Initiative funding for fiscal year 2022-23 to provide additional personnel and operating costs to allow continued support for the California Home Visiting Program. Health Equity Prevention Fund – Children’s Initiative funding supports the implementation of the Yolo County HHSA Public Health Prevention Fund, for the highest risk child in the area of health equity prevention.

Performance Measures included in this Agreement are as follows:
Performance Measures were included in the underlying Agreement and are not being revised by this amendment. (see Att. B. Performance Measures) HHSA confirms YCCA has performed satisfactorily under the current agreement. YCCA has met all deliverables required by the California Department of Public Health (CDPH) for the MIECHV Program. Quarterly staffing reports, semi-annual population surveys/rosters/status reports, and various other annual reports are submitted by YCCA via the CDPH SharePoint monitoring channel. YCCA also collects population data for all participants using model issued forms and reports the data to the County via the Efforts-to-Outcomes (ETO) Software. YCCA has maintained the negotiated caseload capacity and collaborated with County to complete quality improvement projects and activities that align with the CHVP improvement goals. Full integration of the CHVP into the Yolo Local Early Childhood System has been completed.

Competitive Bid Process/Vendor Performance

On May 3, 2019, Yolo County Health and Human Services Agency (HHSA) issued a Request for Proposals (RFP) from qualified community-based organizations to provide Healthy Families America Program. The RFP was open for 42 days (6 weeks), and staff offered potential bidders an opportunity to ask questions during a Mandatory Bidders’ Conference on May 10, 2019.  Bidders were offered the opportunity to submit proposals via the electronic BidSync system or by dropping off a paper proposal directly at HHSA.

HHSA received one (1) proposal(s) from this solicitation.  A Proposal Review Committee was formed that included two (2) HHSA employees from one (1) HHSA branch, and one (1) member from an outside entity.  The proposal submitted by Yolo County Children’s Alliance was selected by all members of the Review Committee and was evaluated and determined to meet all the required qualifications necessary to successfully provide these services.

No general funds are required by this action. This agreement is funded using a combination of California Home Visitation Program (CHVP) funding and Public Health Realignment though the Health Equity Prevention Fund – Children’s Initiative funding. The action increases the contract for FY 2022-23 by $70,000 for a new total contract maximum of $1,393,860.51, for the period of November 1, 2019 through September 30, 2023. The amount of $70,000 is included in the HHSA adopted budget for FY 2022-23 and is part of Public Health Realignment funding.
