Recommended Action

Approve seventh amendment to Agreement 18-218 with BHC Heritage Oaks Hospital to add funding in the amount of $15,050 for a new contract maximum of $3,660,587 for the period of June 1, 2018 through June 30, 2023; increase the Crisis Intervention rate and update and clarify language in the agreement, for the provision of After-Hours Access and Crisis Services.

Reason for Recommended Action/Background

On or about October 9, 2018 the County entered into Agreement with BHC Heritage Oaks for provisions of 24/7 Access Line and After-Hours Crisis Navigation services as an effort to build the crisis continuum in Yolo County, as well as meet state requirements for a 24/7 service access portal. Crisis Navigation services are handled during business hours by HHSA Crisis staff.

The initial service elements included 24/7 Behavioral Health Access & Crisis Line (Scope A.1), Law Enforcement After Hours Crisis Response (Scope A.2), Hospital & Emergency Room After Hours and Crisis Response (Scope A.3).
On or about July 18, 2019 the Parties amended the Agreement via the First Amendment to combine Scopes 2 and 3 as After-Hours Crisis Response Services (Scope A2 and A3). On or about December 16, 2020 the Parties further amended the Agreement via the Second Amendment to update the term of the Agreement through June 30, 2021; update funding, add rates for Fiscal Year (FY) 2020-21 and update and clarify language in the Agreement; and scope A.3 was removed leaving only two service elements as detailed below:
  1. 24/7 Access & Crisis Line Services (Scope A.1)
    Provide triage services to calls coming into the 24/7 Behavioral Health (mental health and substance use services) Access Line for Yolo County Health and Human Services (HHSA) and assess and appropriately disposition service calls, urgent support requests and crisis calls to support the community. These services will be provided 24 hours a day, 7 days per week, 365 days per year.
  2. Crisis Navigation (Scope A.2)
    Provide after-hours service eligibility and medical necessity review as well as appropriate acute inpatient psychiatric placement seeking for Yolo County beneficiaries placed on an involuntary psychiatric hold in local County hospital emergency rooms. 
     
On or about June 29, 2021 the Parties amended the Agreement via the Third Amendment to extend services through Decmeber 31, 2021 and added funding for FY2021-22.

On or about January 13, 2022 the Parties further amended the Agreement via the Fourth Amendment to update the term of the Agreement through June 30, 2022.

On or about July 7, 2022 the Parties further amended the Agreement via the Fifth Amendment to extend the term of the Agreement through September 30, 2022, add funding for FY 2022-23; and update and clarify language in the Agreement.

On or about December 29, 2022 the Parties further amended the Agreement via the Sixth Amendment to extend the term of the Agreement through June 30, 2023 and add funding for FY 2022-23.

This Seventh amendment adds funding in the amount of $15,050 to Scope A.1; increases the Crisis Intervention rate and updates and clarifies language in the Agreement that provides 24/7 behavioral Health Access Line Services. This funding is needed to cover the increased personnel costs the vendor has experienced throughout the pandemic as the Access Line is staffed with Mental Health Clinicians.

Performance Measures included in this Agreement are as follows:
The Performance Measures were included in the underlying Agreement, revised in the Second Amendment and are not being revised at this time (See Att. B. Performance Measures)

HHSA confirms BHC Heritage Oaks Hospital has performed satisfactorily under this agreement. Annual vendor performance data is submitted by the vendor, complementing data generated from HHSA’s electronic health record Avatar regarding beneficiary service access via this Line. Last FY, the vendor handled no less than 10,000 calls in a timely manner.

Competitive Bid Process/Vendor Performance

On December 20, 2017, Yolo County Health and Human Services Agency (HHSA) issued a Request for Proposals (RFP) from qualified community-based organizations to provide After Hours Access and Crisis Services. The RFP was open for 6 weeks, and staff offered potential bidders an opportunity to ask questions during a Mandatory Bidders’ Conference on January 3, 2018.  Bidders were offered the opportunity to submit proposals by dropping off a paper proposal directly at HHSA.

HHSA received two proposals from this solicitation.  A Proposal Review Committee was formed that included three HHSA employees from different branches, and two members from outside entities include behavioral health, emergency services, law enforcement, and the community. The two proposals were evaluated, and because scoring results were very close, it was recommended that individual interviews be conducted to determine the best qualified bidder.

The proposal submitted by BHC Heritage Oaks Hospital was selected as the top choice by all members of the Proposal Review Committee and was evaluated and determined to meet all the required qualifications necessary to successfully provide these services.
