Reason for Recommended Action/Background
Yolo Community Care Continuum (YCCC) Safe Harbor is a Crisis Residential Treatment Facility that provides short–term residential treatment options for clients in behavioral health crisis that can effectively prevent a voluntary/involuntary psychiatric hospital stays and who meet the Medi-Cal mental health medical necessity criteria. Safe Harbor also provides a more supportive step-down option after a inpatient psychiatric hospital stay to help prevent the return of a crisis and support a more stable return to the community and outpatient behavioral health services.
On or about July 7, 2020 the County entered into a 3-year agreement with YCCC for the term of July 1, 2020 through June 30, 2023 with a contract maximum of $2,400,000, for the provision of crisis residential – safe harbor services. The Agreement included 2 optional extension terms for FY 2023-24 and FY 2024-25 in an amount not to exceed $800,000 per fiscal year.
On or about September 2, 2022 the Parties amended the Agreement via the First Amendment to update and clarify language in the Agreement.
HHSA request approval of this amendment to add funding in the amount of $105,000 for FY 2022-23 for a new contract maximum of $2,505,000; and update and clarify language in the agreement. The additional funding is to cover remaining costs for client services.
A formal solicitation for these services (as part of the County’s move towards the Crisis Now model) is planned to be released in Spring FY 2022-23 for a targeted contract start date of January 1, 2024. However, due to County procurement delays, this extension provides for any unexpected RFP process delays which would in turn delay new vendor start until later in FY23/24.
Performance Measures included in this Agreement are as follows:
The Performance Measures were included in the underlying Agreement and are not being revised at this time (See Att. B. Performance Measures)
HHSA confirms Yolo Community Care Continuum has performed satisfactorily under this agreement. After the most recent contract revision, YCCC’s performance has improved and the day-to-day operational relationship and communication between the vendor and HHSA has improved. Further the vendor has not denied any referrals (which was a prior problem), nor has any clients stayed too long at the facility (and the mean length of stay has dropped to 8 days).

Competitive Bid Process/Vendor Performance
On August 31, 2020, the then, Yolo County Procurement Manager or designee approved a sole/single source procurement for these services.
Procurement reviewed HHSA’s request and determined that it was in the best interest of the County to approve the single/sole source, based on satisfactory service/reasonable prices and to avoid the interruption of County business. Without the sole source approval and an agreement with the vendor, the County would not have a local crisis residential facility and beds to support clients in crisis and/or post crisis. Factors considered, included but were not limited to:
1) obtaining local short-term residential treatment options for clients in psychiatric crisis which can effectively prevent voluntary/involuntary hospital stays; 2) competitive pricing and long-term cost savings keeping clients out of higher priced facilities like a Psychiatric Health Facility (PHF); and 3) ease of geographical access to 24/7 crisis residential services for clients throughout Yolo County as well as ongoing HHSA service access for both new and ongoing clients. This vendor facility is only 1 mile away from HHSA’s main Woodland clinic which allows HHSA staff to provide medication support services, immediate crisis assessment services, and quick access to the face-to-face services.

No general funds are required by this action. These services will be funded by MHSA. The action increases the contract maximum by $105,000 for fiscal year 2022-23 for a total of $2,505,000 for the period of July 1, 2020 through June 30, 2023. The amount of $105,000 is included in the HHSA adopted budget for fiscal year 2023-24. The related funding will be included in the requested budget process for future fiscal years.

In no event shall the term of the Agreement extend beyond December 31, 2023, nor shall the total contract maximum exceed the amount of TWO MILLION NINE HUNDRED FIVE THOUSAND DOLLARS ($2,905,000), unless otherwise agreed to in writing by the parties and in conformity with the then-current Yolo County Procurement Policy approved by the Yolo County Board of Supervisors.
