Approve first amendment to Agreement No. 23-251 with Yolo County Children’s Alliance to extend the term of the agreement through December 31, 2024; and add funding in the amount of $452,245.50 for a new contract maximum of $1,356,736.50 for the period of October 1, 2023 through December 31, 2024; and add one (1) optional six (6) month extension for 2024-25; and add one (1) additional twelve (12) month extension for 2025-26 for the provision of CA Home Visitation program funded through the California Home Visiting Program (CHVP) grant. (No general fund impact) (Sullivan)

Recommended Action

  1. Approve first amendment to Agreement No. 23-251 with Yolo County Children’s Alliance to extend the term of the agreement through December 31, 2024; and add funding in the amount of $452,245.50 for a new contract maximum of $1,356,736.50 for the period of October 1, 2023 through December 31, 2024; and add one (1) optional six (6) month extension for 2024-25; and add one (1) additional twelve (12) month extension for 2025-26 for the provision of CA Home Visitation program funded through the California Home Visiting Program (CHVP) grant; and 
     
  2. Delegate authority to the Director of the Yolo County Health and Human Services Agency or her/his designee to: 1) exercise optional extensions up to $452,245.50 for fiscal 2024-25 and up to $904,491 for fiscal year 2025-26; and 2) issue related option year notices. Please see Section 1.B of Att. A. Agreement.

Reason for Recommended Action/Background

The Patient Protection and Affordable Care Act of 2010 established the Maternal, Infant and Early Childhood Home Visiting (MIECHV) Program to provide an opportunity for collaboration and partnerships at the federal, state, and community levels to improve outcomes for families who reside in at-risk communities through evidence-based home visiting programs.

The California Home Visiting Program (CHVP) strives to develop collaborative community systems that protect and improve the health and developmental outcomes for California’s pregnant women, parents, and families. 

Approval of this Amendment will allow a continuation of services that provides the parameters for implementing or expanding an existing Nurse-Family Partnership (NFP) or Healthy Families America (HFA) home visiting program in accordance with Federal MIECHV and State requirements to achieve positive outcomes for each of the following five goals:
  1. Provide leadership, coordinate maternal and early childhood systems & supports to advance federal, state, and local efforts to improve the health and well-being of families in California
  2. Cultivate strong communities
  3. Promote maternal health and well-being
  4. Improve infant and child health development
  5. Strengthen family functioning

Performance Measures included in this Agreement are as follows:
The Performance Measures were included in the underlying Agreement and are not being revised at this time (See Att. B. Performance Measures).

Competitive Bid Process/Vendor Performance

On April 20, 2023, the County via the Department of General Services, Procurement Division (DGS-Procurement) [formerly Department of Financial Services, Procurement Division] issued a Request of Proposal (RFP) on behalf of the Health and Human Services Agency (HHSA) for the Healthy Families America Program. The solicitation was open for 55 days (just over 7 weeks). On May 17, 2023, DGS and HHSA offered potential bidders an opportunity to ask questions during a bidder’s conference. 

The County received 1 (one) proposal from this solicitation. DGS reviewed the proposal for minimum requirements and moved it forward to the Review Committee, comprised of three County HHSA staff.

The Review Committee evaluated the proposal and determined that the proposal submitted by Yolo County Children’s Alliance met all the required qualifications necessary to successfully provide these services.  On July 26, 2023, the County issued an Intent to Award letter.

No general funds are required by this action. These services will be funded by California Home Visitation Program (CHVP)- MIECHV and SGF EBHV. The total amount of this action is $452,245.50 for the period of July 1, 2024 through December 31, 2024. The amount of $452,245.50 is included in the HHSA Recommended Budget for 2024-25.
