Recommended Action

  1. Approve first amendment to Agreement No. 23-317 with C.O.R.E Medical Clinic, Inc., to add funding in the amount of $120,000 for 2024-25 for a new contract maximum of $1,825,000 for the period of January 1, 2024 through June 30, 2025; and add rates for 2024-25; and authorize the Procurement Manager or designee to execute an amendment to add $122,030.16 for 2023-24 for the provision of providing substance use disorder (SUD) adult continuum of care services.
     
  2. Delegate signing authority to the Yolo County Procurement Manager or designee to sign the second amendment to Agreement No. 23-317 to add $122,030.16 for 2023-24.

Reason for Recommended Action/Background

The County entered into an agreement with the California Department of Health Care Services (DHCS) to provide Drug Medi-Cal Organized Delivery System (DMC-ODS) Substance Use Disorder (SUD) treatment services to eligible clients. DMC-ODS is a program for organized SUD services to Medi-Cal eligible individuals with SUD residing in a county that has elected to participate in the DMC-ODS. The program provides beneficiaries with access to care and system interaction needed to achieve sustainable recovery. The County also receives funding from a variety of other federal and state funding streams for the provision of SUD treatment services to various client populations, which may include but not limited to:  Substance Abuse Prevention and Treatment (SABG) Block Grant Funds; Substance Abuse and Mental Health Services Administration (SAMHSA); CalWORKS and Child Welfare Services (CWS) funding; 2011 Realignment; AB109 Revenue, FFP, and Opioid Settlement funds.

The SUD Continuum of Care services includes the following:
  • Residential treatment services
  • Outpatient and intensive outpatient treatment services 
  • Residential withdrawal management (detoxification) services
  • Care Coordination services
  • Recovery services
  • Clinician Consultation
  • Recovery Residence
  • Narcotic treatment program services/opioid treatment program
These services are provided to any eligible Yolo County resident referred through an access point established by Yolo County or referred by provider and include, but not be limited to, the following populations/programs:
  • CalWORKS Participants
  • Individuals with an open Child Welfare Services (CWS) Case Plan
  • Individuals who are Pregnant or Postpartum (Perinatal Services)
  • Individuals who are involved with the Criminal Justice and those partially funded by system (AB109)
  • Medically Indigent Adults
  • Individuals with Co-Occurring substance use and mental health disorders and SUD disorders.
As a result of competitive solicitation process (details below),C.O.R.E. Medical Clinic, Inc (C.O.R.E.) was selected to provide opioid/narcotic treatment program (OTP/NTP) services in narcotic treatment program licensed facilities, care coordination, and clinician consultation services. On or about February 7, 2024, the County and C.O.R.E. entered into Agreement No. 23-317 (“the Agreement”) for these services. On or about May 8, 2024, the County exercised its option to extend the Agreement per its terms for fiscal year 2024-25. The services that C.O.R.E. provides are crucial to helping Yolo County meet the needs of our residents who struggle with substance use and need OTP/NTP services as part of their continuum of care.

Health and Human Services Agency (HHSA) requests approval of this amendment to the Agreement to add funding and to update the rates for fiscal year 2024-25 to enable HHSA to continue providing these crucial services. The additional funding in FY2024-25 is needed to serve Yolo County clients with medication assisted treatment and medications to recover from a substance use disorder.

HHSA is also requesting delegated signing authority to the Yolo County Procurement Manager, or designee, to sign the second amendment to Agreement No. 23-317 to add funding for fiscal year 2023-24. Due to an increased utilization in services, the costs for fiscal year 2023-24 were greater than projected and there are pending claims. In order to expedite the processing of the second amendment so these payments can be made, HHSA requests delegated signing authority to the Yolo County Procurement Manager. The first amendment was already in process and signed by County Counsel and C.O.R.E. when the 2023-24 pending claims issue was identified. HHSA elected not to pull back the first amendment to incorporate the 2023-24 increase, so that 2024-25 payments would not be delayed. The funding for the 2023-24 increase was found in cost savings from other SUD treatment service agreements.

The Performance Measures were included in the underlying Agreement and are not being revised at this time (see Att. B. Performance Measures).

HHSA confirms C.O.R.E. Medical Clinic, Inc. has performed satisfactorily for the County. During the period of July 1st through December 31st of 2023 (services provided under the previous agreement. with C.O.R.E—Agreement No. 23-120), 71 clients were enrolled in the program with at least 97% of the enrolled clients receiving at least 2 or more services within 30 days of a diagnosis being established by the program. Of the 71 clients, 65 were referred or linked to other needed services (MH, PCP, other community programs). This shows that they are treating the whole person. During treatment, there were zero days of client incarceration.

Competitive Bid Process/Vendor Performance

On March 3, 2023 the County via the Department of General Services, Procurement Division (Procurement) issued a Request for Proposal (RFP) on behalf of the HHSA for Substance Use Disorder Adult Continuum of Care services. The solicitation was open for 64 days, (just over 9 weeks). On March 24, 2023, the County offered potential bidders an opportunity to ask questions during a bidder’s conference. 

The County received three (3) proposal(s) from this solicitation.  Procurement reviewed the proposal for minimum requirements and all three (3) proposals were moved forward to the Review Committee for scoring. The Review Committee was comprised of two (2) HHSA staff members, one (1) Yolo County Probation staff member, and one (1) Placer County staff member.

The Review Committee evaluated the proposals and determined that the proposal submitted by C.O.R.E. met all the required qualifications necessary to successfully provide these services.  On or about June 27, 2023, the County issued an Intent to Award letter. Additional information regarding the Competitive Bid Process is provided below, including a summary of the solicitation evaluation criteria.

First Amendment: No general funds are required by the first amendment action which increases fiscal year 2024-25 by $120,000 for a new fiscal year 2024-25 amount of $1,200,000. These services are funded by Behavioral Health (BH) 2011 Realignment, FFP, State General Fund, Child Welfare Title IV-E, AB109 Revenue, CalWORKs Mental Health and Substance Abuse Allocation, and Opioid Settlement funding. The amount of $120,000 is included in the HHSA recommended budget for fiscal year 2024-25.

Second Amendment: No general funds will be required by the second amendment action which will increase fiscal year 2023-24 by $122,030.16 for a new fiscal year 2023-24 amount of $747,030.16. The amount of $122,030.16 was included in the HHSA adopted budget for fiscal year 2023-24.

This agreement also includes three (3) additional twelve (12) month periods in an amount less than or equal to $1,080,000 per fiscal year and one (1) additional six (6) month optional period. The funding for these option years will be included in the requested budget process for future fiscal years. There are no changes being made to the optional extension amounts. These optional extension amounts were already approved by this Board. The increases via the first ($120,000) and second amendment ($122,030.16) will modify the lifetime maximum amount of the agreement from $5,485,000 to $5,727,030.16.

In no event shall the term of the Agreement extend beyond December 31, 2028, nor shall the lifetime maximum amount of the agreement exceed the amount of FIVE MILLION SEVEN HUNDRED TWENTY-SEVEN THOUSAND THIRTY DOLLARS AND SIXTEEN CENTS ($5,727,030.16), unless otherwise agreed to in writing by the parties and in conformity with the then-current County Procurement Policy.
