Recommended Action
Approve second amendment to agreement No. 23-320 with Yolo Community Care Continuum to add funding in the amount of $45,417.79 for 2023-24 for a new contract maximum of $3,275,667.79 for the period of January 1, 2024 through June 30, 2026, for the provision of Adult Crisis Residential Services at Safe Harbor in compliance with the new California Advancing and Innovating Medi-Cal (CalAIM) initiative.

Reason for Recommended Action/Background
Crisis Residential Treatment Services means therapeutic or rehabilitative services provided in a non-institutional residential setting which provides a structured program as an alternative to hospitalization for clients experiencing an acute psychiatric episode or crisis who do not have medical complications that would require nursing care. The service includes a range of activities and services that support beneficiaries in their efforts to restore, maintain, and apply interpersonal and independent living skills, and to access community support systems. The service is available 24 hours a day, seven days a week. HHSA has expanded the breadth of Crisis Residential Treatment in Yolo County with the addition of Short-Term beds (for stays of up to 4 days) for intensive crisis de-escalation services to the existing Longer-Term beds (for stays up to 30 days) for clients in need of prolonged interventions. This is part of HHSA’s efforts to adopt a version of the Crisis Now model.
As a result of a competitive solicitation process (details below) Yolo Community Care Continuum was selected to provide Adult Crisis Residential Services. On or about December 8, 2023, the County entered into an Agreement with Yolo Community Care Continuum for the provision of adult crisis residential services at Safe Harbor in an amount of $3,230,250 for the period of January 1, 2024 through June 30, 2026. The Agreement included 2 optional extensions for Fisal Year’s 2026-27 and 2027-28. On or about August 24, 2024, the Agreement was amended via the First Amendment to identify the funding sources.
Now Health and Human Services Agency (HHSA) requests approval of this second amendment to the Agreement to add MHSA funding for 2023-24 to cover remaining year end claims.
Performance Measures
The Performance Measures were included in the underlying Agreement and are not being revised at this time (See Att. B. Performance Measures).
HHSA confirms Yolo Community Care Continuum has performed satisfactorily under this agreement. During their stay at Safe Harbor Crisis Treatment program, 80% of clients demonstrated therapeutic program engagement and 75% of clients met their individual short-term goals during their stay. On average, all clients served stabilized within 24 days and 80% of clients served in FY 2023-24 did not evidence the need for psychiatric hospitalization within 6 months after a Safe Harbor stay.

Competitive Bid Process/Vendor Performance
On June 28, 2023, the County via the Department of General Services, Procurement Division (DGS-Procurement) [formerly Department of Financial Services, Procurement Division], issued a Request for Proposal (RFP) on behalf of the Health and Human Services Agency (HHSA) for Crisis Residential Services. The solicitation was open for 34 days, (just under 5 weeks). On July 11, 2023, DGS and HHSA offered potential bidders an opportunity to ask questions during a bidder’s conference.
The County received one (1) proposal from this solicitation. DGS reviewed the proposal for minimum requirements and moved it forward to the Review Committee, comprised of five (5) evaluators, four (4) County Staff and one (1) from outside the County.
The Review Committee evaluated the proposal and determined that the proposal submitted by Yolo Community Care Continuum met all the required qualifications necessary to successfully provide these services. On September 18, 2023, the County issued an Intent to Award letter.

No general funds are required by this action. The increase funding will be funded by MHSA. The action increases the contract maximum by $45,417.79 for a total of $3,275,667.79 for the period of January 1, 2024 through June 30, 2026. The amount of $45,417.79 is included in the HHSA adopted budget for fiscal year 2023-24. The related funding will be included in the requested budget process for future fiscal years.

In no event shall the term of the Agreement extend beyond June 30, 2028, nor shall the total contract maximum exceed the amount of FIVE MILLION EIGHT HUNDRED FIFTY-NINE THOUSAND EIGHT HUNDRED SIXTY-SEVEN DOLLARS AND SEVENTY-NINE CENTS ($5,859,867.79), unless otherwise agreed to in writing by the Parties and in conformity with the then-current Yolo County Procurement Policy approved by the Yolo County Board of Supervisors.
