Approve agreement with RG Legacy II, LLC dba Pasadena Nursing Center in the amount of $1,139,676 for the period of April 1, 2025 through June 30, 2028 for the provision of 24-hour supervised clinical and nursing care services to long-term consumers with severe mental illness (SMI). Approve third amendment to Agreement No. 23-315 with Ole Health dba CommuniCare OLE to add funding in the amount of $605,000 for 2025-26 for a new contract maximum of $5,707,806.84 for the period of January 1, 2024 through June 30, 2026; for the provision of Substance Use Disorder Adult Continuum of Care services. (No general fund impact) (Morales)
23.
Adopt resolution authorizing the Yolo County Health and Human Services Agency to participate in and receive Transitional Housing Program – Round 7 funding in the amount of $119,904 and Housing Navigation Maintenance Program – Round 4 funding in the amount of $66,979 from the California Department of Housing and Community Development to provide housing services and activities to target populations in the community; and adopt budget resolution amending the Health and Human Services Agency budget for fiscal year 2025-26 to reflect an increase of $186,883 to the Child, Youth, & Family Child Welfare Transition Age Youth cost center. Approve third amendment to Agreement No. 21-139 with Mental Health Management I, Inc., dba Canyon Manor, to reduce funding by $175,000 for 2024-25, extend the contract term through June 30, 2026, add $1,050,000 for 2025-26 for a new total contract amount of $4,775,000 for the period of July 1, 2021 through June 30, 2026, update rates for 2024-25 and 2025-26, and update County email addresses throughout the agreement for mental health rehabilitation center (MHRC) residential treatment services.
