Resolution - Public Health Services:
Consider a resolution authorizing the Mayor to execute Contract 16909, with Unite USA, Inc., for a technology platform to assist the Lubbock Health Department in screening clients for Social Determinants of Health.
Item Summary
This contract is for a new technology platform to continue the work of the LBK Community network. The network is a collaborative alliance of over 30 community-based organizations in Lubbock who are committed to working together to improve the lives and well-being of the community members they serve.
The goals of the network are to:
Increase collaboration
Improve health outcomes
Streamline processes
Increase data collection for grant reporting and funding
RFP 23-16909-YB was issued for services for a technology platform to assist the Lubbock Health Department in screening clients for Social Determinants of Health as well as referring at-risk clients to appropriate social services.
The following Contractors submitted proposals which were received and opened on November 22, 2022.

Staff and the Evaluating Committee recommend award of the contract to Unite USA, Inc., of New York, New York, for $137,500.
The contract term will be 1 year, with the option of 4 one-year extensions, with the term beginning upon formal approval. All stated annual quantities are approximations of usage during the time period to be covered by pricing established by this bid. Actual usage may be more or less. Order quantities will be determined by actual need. The City of Lubbock does not guarantee any specific amount of compensation, volume, minimum, or maximum amount of services under this bid and resulting contract.
The contractor must maintain the insurance coverage required during the term of this contract including any extensions. It is the responsibility of the Contractor to ensure that valid insurance is on file with the Purchasing and Contract Management Department as required by contract or contract may be terminated for non-compliance.
Fiscal Impact
The cost in Year 1 of this contract is $197,500. In Years 2, 3, 4, and 5, the annual cost will be $137,500 and will be funded by the COVID-19 Health Disparities Grant.
