Item Summary
The funding from this contract is intended to help achieve health equity by addressing the Social Determinants of Health (SDOH), such as systemic racism, poverty, poor access to quality healthcare, transportation, housing insecurity, nutrition insecurity, commercial tobacco-free policies, safe spaces for physical activity, and social connectedness. Through a community health needs assessment, multi-sector partnerships and improvement planning, at the federal, state, local, tribal and territorial governments level, can invest in long-range strategies to address and improve SDOH in communities with the poorest health outcomes.

Total award amount is $125,000. Cost sharing or matching funds are not required for this contract.
