Approve the issuance of the replacement tax refund check in the amount of $429.63 to Judith Mikeal of St. David, AZ.
Background:
Ms. Mikeal was issued a check for $429.63 on June 6, 2011. Because of several deaths in her family, she traveled outside of AZ for a number of months. Then she was faced with flooding problems and did not discover this check until recently. Because the check is more than a year old, the Treasurer's Office cannot issue a new check without the approval of the Board of Supervisors.
