Approve ADHS16-109191 Amendment 7 for Healthy People Healthy Communities, between the Arizona Department of Health Services and Cochise Health & Social Services, in the amount of $684,356, effective July 1, 2019 through June 30, 2020.
Background:
In order to best implement the goals of the Arizona State Health Improvement Plan (AzHIP), ADHS developed a process to align strategies and improve integration of public health prevention programs. Multiple bureaus within ADHS collaborated on the development of this Intergovernmental Agreement (IGA) which combines seven programs into one shared IGA. Programs included in this IGA address several health priorities from the AzHIP, including but not limited to: cardiovascular disease, chronic lower respiratory diseases, diabetes, unintentional injury, obesity, teen pregnancy, tobacco, and access to well care. This IGA includes opportunities to address cross-cutting strategies, such as enhancing the physical and built environment, school health, and worksite wellness.

- new evidence-based strategies and deliverables for all program areas
- a new price sheet for 07/01/2019 through 06/30/2020
