Approve Contract Amendment 4 CTR050594 Health Start Program, between the Arizona Department of Health Services and Cochise County Health & Social Services (CHSS), in the amount of $353,000, for the period July 1, 2023, through July 5, 2024.
Background:
CTR050594 Amendment 3 was approved by the BOS on July 25, 2023. This amendment was only program renewal.
Amendment 4 includes the price sheet with an annual allotted amount of $353,000. In addition, A4 changes from a Fee for Service IGA to a Cost Reimbursement IGA.
CCHSS Health Start Program will continue providing educational services and resources to pregnant women and families with children under age two by utilizing the curriculum “Partners for a Healthy Baby”. These services are provided via in-home visits, at a public location, and/or at any of the CCHSS office.
The Health Start Program serves approximately 250 families, providing an average of 1,500-2,000 intervention visits, throughout Cochise County.

Impact of NOT Approving/Alternatives:
Not approving this item would result in the termination of the CCHSS Health Start contract which provides services to at-risk pregnant women and children under the age of 2 throughout Cochise County.
