Agenda Item Text:
Approve Amendment 4 to Intergovernmental Agreement (IGA) CTR055282, Healthy People Healthy Communities, between the Arizona Department of Health Services and Cochise County Health & Social Services, in the amount of $631,421, effective July 1, 2024, through June 30, 2025.
Background:
The Healthy People Health Communities IGA was implemented by the Arizona Department of Health Services to best apply the goals of the state’s Health Improvement Plan (AzHIP). This IGA combines several programs into one shared IGA – Tobacco Prevention, Health in Az Policy Initiative and Chronic Disease Prevention, Teen Pregnancy Prevention, and Suicide Mortality Review Program. The original IGA is into its 10th year. This IGA serves people of all ages – youth, teens, and adults. The programs address several health priorities, including but not limited to: cardiovascular disease, chronic lower respiratory diseases, Alzheimer’s, cancer, obesity, teen pregnancy, tobacco and vaping use, worksite wellness, school health wellness, and suicide mortality review. Outreach is provided via group workshops, health fairs, classroom education, policy assistance development, and fatality review. This IGA currently funds 8 county positions. The total budget for all programs within the IGA is $631,421.
One major change this year, our program will compile case summaries and conduct suicide mortal reviews for Graham, Greenlee, and Santa Cruz Counties in addition to Cochise. Because of this, we are receiving an additional amount of $75,000.

Department's Next Steps (if approved):
Program will continue and services will be delivered.
Impact of NOT Approving/Alternatives:
Not approving this item would result in the termination of the Cochise County Health Department Healthy People Healthy Communities contract which provides services to improve the health and assist in preventing chronic conditions of Cochise County residents, and SMR reviews would need to be conducted at the state level.
To BOS Staff: Document Disposition/Follow-Up:
Send signed IGA to CCHSS, Suzanne Hagle, Prevention Services Division Director.
