Approve ADHS16-109191 Amendment 5, Healthy People Healthy Communities, between the Arizona Department of Health Services and Cochise Health & Social Services, for the period of 07/01/2018 to 6/30/2019 in the amount of $643,670.

Background:

In order to best implement the goals of the Arizona State Health Improvement Plan (AzHIP), ADHS developed a process to align strategies and improve integration of public health prevention programs.  Multiple bureaus within ADHS have collaborated on the development of this Intergovernmental Agreement (IGA) that combines seven programs into one shared IGA.  Programs included in this IGA address several health priorities from the AzHIP, including but not limited to, cardiovascular disease, chronic lower respiratory diseases, diabetes, unintentional injury, obesity, teen pregnancy, tobacco and access to well care.  This IGA includes opportunities to address cross-cutting strategies, such as enhancing the physical and built environment, school health and worksite wellness.
 
This amendment provides for the following:
  • a revised Scope of Work
  • new evidence-based strategies for HAPI & Chronic Disease; and
  • a new price sheet for 07/01/2018 through 06/30/2019.

Not approving this amendment would result in the termination of all included CHSS contracts that provide for the services described above, to improve the health of Cochise County residents.
