Approve Amendment #5 to the Emergency Preparedness Program Contract #ADHS17-133164 between the Arizona Department of Health Services and Cochise Health and Social Services in the amount of $180,975, effective September 1, 2018 through August 31, 2019.
Background:
| With this one-time funding opportunity, CHSS will (i) staff a licensed clinical social worker as a case manager at the county jail who will be responsible for case management for transitioning opioid-addicted individuals to the community, and (ii) contract with a local EMS agency to provide a link in community after individuals are discharged from ER after an opioid overdose. This partnership will provide a paramedic home visit after discharge, addressing resources, linkages to care, and facilitating for needs experienced after overdose. This incremental funding is non-recurring and all funds must be expended by 8/31/19. |
