Approve Subaward Agreement 107-23 between SouthEastern Arizona Governments Organization (SEAGO) and Cochise Health & Social Services for Area Agency on Aging (AAA) Case Management services, in the amount of $61,667, effective July 1, 2022 through October 31, 2022.

Background:

Cochise Health and Social Services (CHSS) through its Area Agency on Aging (AAA) Program provides Case Management services to both care recipients and unpaid family caregivers.  Services are generally restricted to older individuals aged 60 or older per SEAGO Service Specifications.  Case Managers conducts client in-home assessments, provide referrals for home delivered meals, housekeeping and personal care services funded by SEAGO through contracts with other vendors and link clients to services available in the community.  Case Management services enables elderly and disabled county residents to remain in their homes longer, living independently with supports for as long as safely possible.

The shorter term of this contract is to allow for the transition of case management services to be provided in-house by SEAGO employees effective November 1, 2022.  The case management service will cease to be subcontracted to Cochise County as a vendor.  This transition is in line with SEAGO’s Five-Year Strategic Plan FY2021-2025.

Impact of NOT Approving/Alternatives:

Closure of services for 450 county residents without the benefit of a transition to SEAGO.  Closure of case management service would contribute to increased enrollment on ALTCS (Arizona Long Term Care System).
