Approve Amendment 5 of Contract CTR050594 between the Arizona Department of Health Services (AHDS) and Cochise County Health & Social Services (CHSS) for the Health Start Program, in the amount of $353,000, effective July 1, 2024, through July 5, 2025.
Background:
Cochise County Health and Social Services (CCHSS) has received the Health Start grant for over 35 years. This program serves pregnant women and families with children up to the age of 2. The CCHSS Health Start Health Educators are trained to deliver the Arizona Department of Health Services (ADHS) state required curriculum, “Partners for a Healthy Baby” to support and educate parents on how to raise healthy children safely. The curriculum and the education provided by our Health Educators aims to reduce pregnancy and post-partum risks through a community health worker model. Pregnant women and parents receive guidance and information including what to expect during pregnancy, breastfeeding education and support, nutrition for children and pregnant women, screening for child development, activities to support healthy development, and home health and safety. These services are provided mainly through in-home visits, or at a public location such as a park/playground, a library, etc.
The 4 Health Start Program Health Educators have a caseload of 25 families, and provides an average of 3 interventions to each family, monthly.
This is a grant-funded, Cost Reimbursement IGA. The grant supports salary, employee related expenses and expenses for 5.5 full-time employees. It also supports educational materials and resources for the families enrolled in the program.

Impact of NOT Approving/Alternatives:
Not approving this item would result in the termination of the CCHSS Health Start contract which provides services to at-risk families, pregnant women, and children throughout Cochise County.
